  • Welcome to the St. Patrick Parish Athletics registration for 2025-26! 

    Please fill out this form completely for each student.

    Please note that athletics payments can only be made via credit or debit card through PayPal. This is to ensure safety and accountability. There are no additional fees charged for this service.

    Parish Athletics is open to anyone who is eligible, regardless of their ability to pay. If there is a financial hardship, please contact the athletic director to discuss the situation prior to registration.

  • Each athlete is required to have a physical examination, by a licensed medical doctor, prior to participation in practice or competitions.  This physical examination must be dated within the year prior to completion of the sport season (i.e. the physical must not be more than a year old).

    Afer you submit your registration, you'll receive some more information on physicals and where to get the forms.

  • I hearby certify that the student-athlete, parent and medical information provided is complete and true to the best of my knowledge.

    Furthermore, I have read and understood the information regarding athletic physical requirements, as outlined above. I understand that failure to provide a physical form that meets the requirements by the due date will result in my chld being unable to participate in parish athletics. Fees will not be refunded.

  • 2025-26 EMERGENCY MEDICAL AUTHORIZATION

    THIS INFORMATION IS REQUIRED BY THE STATE OF OHIO AND MUST BE COMPLETED IN FULL
  • PERSONS TO BE NOTIFIED IF PARENTS CANNOT BE REACHED

    These persons have been given permssion to authorize emergency treatment for children who become ill or injured while participating in an athletic events when parents cannot be reached.  You must add at least one person (other than parent or guardian) and may add multiple persons.

  • EMERGENCY CONSENT TO TREAT

    In the event reasonable attempts to contact parent(s) or emergency contact(s) are unsuccessful, we require authorization for medical professional(s) or hospital(s) to treat the student-athlete in an emergency situation, if that is desired.

      1. The administration of any treatment deemed necessary by the following physician and/or dentist or in the event the designated practitioner is not available, by another licensed physician or dentist; and

      2. Permission to be transported to the preferred hospital listed below or any hospital reasonably accessible during an emergency situation.
    • This authorization does not cover major surgery unless the medical opinions of two other licensed physicians or dentists, concurring in the necessity for such surgery, are obtained prior to the performance of such surgery.

  • PARENT AND STUDENT AGREEMENTS

    Please read each agreement fully, check the box below each agreement, and then provide digital signatures where prompted.
  • CYO ELIGIBILITY AND AUTHORIZATION STATEMENT

  • We, athlete and parent, understand that participation in athletics involves the possibility of a serious or even fatal injury. In consideration for our child’s opportunity to participate in this program, we, the parents, individually and on behalf of our child, expressly assume any and all risks associated with and arising from such participation, including, but not limited to possible exposure to and/or infection with COVID-19, bodily and emotional injury, at practice, competitive events, and any other related activity, including transportation to and from any event by a volunteer.

    We hereby release the Diocese of Toledo, CYO, St. Patrick Parish, the Northwest Ohio Catholic Parish League, any parish and/or school sponsor and all of their agents from any and all liability for any such injury or damage.

    We acknowledge that we have provided the required emergency medical information and authorization as part of this online registration.

    We will abide by CYO rules, the Parents’ Code of Ethics and the direction of CYO and game officials. We also grant permission to the Diocese of Toledo, CYO, St. Patrick Parish or their agents to take photographs of my children and use them as they deem necessary.

  • CYO PARENTS' CODE OF ETHICS

  • I hereby pledge to provide positive Christian support, care and encouragement for my child participating in the youth athletic program by following this Parents’ Code of Ethics Pledge. 

    • I will place the emotional and physical well-being of my child ahead of any personal desire to win.
    • I will demonstrate the Christian values of self-restraint, fair play and sportsmanship in my treatment of others at every game, practice and other CYO event.
    • I will ask my child to treat all players, coaches, fans and officials with respect regardless of race, sex or ability.
    • I will not leave my child at any of the facilities until I am sure one of my child’s coaches has arrived. Also, I will make sure I am prompt in picking my child up after each practice session.
    • I will demand a drug, alcohol and tobacco-free environment for my child and agree to assist by refraining from their use at all CYO events.
    • I will do my best to make by child’s involvement with youth athletics a positive experience, while always remembering that the game is for the youth not the adults.
    • I will ensure that my child is free from symptoms of illness before allowing him/her to attend a practice or competition.
  • 24-HOUR RULE AND CONTACT PROCEDURE

  • Win or lose, youth sporting events can become emotionally charged, so we have implemented a "24-hour rule" and contact procedure for all teams at St. Patrick:

    There shall be no contact (verbal, phone, email, text, etc.) with any member of the coaching staff or athletic director regarding a complaint or issue of concern (whether it is about a Coach or player's conduct, language, playing time, position played, etc...) before, during or after a game or practice for a 24-hour period following the occurrence of an issue of concern.

    Following the 24-hour period the following steps should be taken:

    • Athlete verbally communicates with coach. If the situation is not resolved...
    • Athlete and parent then speak to coach together. If not resolved...
    • Athlete and parent have a meeting with coach and AD. If the situation still is not resolved...
    • All parties have a meeting with the principal and/or pastor.

  • CYO ATHLETE'S CODE OF ETHICS

  • I hereby pledge to be positive about my athletic experiences and accept responsibility for my participation by following this Athlete’s Code of Ethics Pledge.

    • I will demonstrate the Christian values of self-restraint, fair play, and sportsmanship in my treatment of others at every game, practice, and other CYO event.
    • I will do my very best to listen and learn from my coaches and to use my God given abilities to their fullest extent to help the team.
    • I will treat all players, coaches, fans and officials with respect regardless of race, sex, creed, or abilities and I will expect to be treated accordingly.
    • I will respect the property of others, as well as, the public facilities we use for practices and for games.
    • I will make sure I am not dropped off at the facility until I know one of my coaches has arrived nor remain there after my coaches have left the facility.
    • I deserve to play in an environment that is free of drugs, tobacco, and alcohol and expect adults to refrain from their use at all youth athletic events.
    • I will remember that athletics is an opportunity to have fun and to learn how to win and lose with dignity. 

    Failure to uphold any of these statements may lead to disciplinary action by the Parish or CYO Office, which may include, but is not limited to, the forfeiture of the privilege of participating in CYO and parish athletic events.

    • All paperwork, physicals and fees must be complete by the first practice for an athlete to participate.

    • FINAL INFORMATION

    • Be sure you have filled out all appropriate items in the online form.

      On the next screen, please select the checkboxes for each sport in which your child plans to participate, and then select the age group for which you are registering. You may also consider an athletic booster donation. The online form will automatically calculate the total due.

      Please note that athletics payments can only be made via credit or debit card through PayPal. This is to ensure safety and accountability. There are no additional fees charged for this service. If a sport is not played this year, all fees for that sport will be refunded.

      When you click submit, your information will be transmitted to St. Patrick and you will get an email confirmation.

      Please note that if you did not already upload an eligible physical form as part of the registration process, you will need to submit a paper copy to the school office. All physical forms must be received before the individual season deadlines previously listed, or the student will not be allowed to participate and registration fees will not be refunded.
