Liability Waiver and Photo/Video Release Form. I, [Parent/Guardian], the undersigned, hereby acknowledge and agree to the following terms and conditions regarding my child's participation in dance classes, activities, events, and performances at Rhythm City Dance by Chelsea LLC. Assumption of Risk: I understand that dance activities involve physical exertion and potential risks, including but not limited to, personal injury, illness, or damage to personal property. I accept all risks associated with my child's participation in dance classes and related activities. Release of Liability: In consideration of my child being allowed to participate in dance classes and activities at Rhythm City Dance by Chelsea LLC, I, on behalf of myself, my child, and our respective heirs, executors, administrators, and assigns, hereby waive, release, and forever discharge Rhythm City Dance by Chelsea LLC, its owners, instructors, employees, and representatives from any and all liabilities, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained by my child or me while participating in dance activities. Photo and Video Release: I grant Rhythm City Dance by Chelsea LLC and its authorized representatives the irrevocable right to use photographs, videos, and audio recordings of my child taken during dance classes, activities, events, and performances for promotional, advertising, and educational purposes, including but not limited to the studio's website, social media, brochures, newsletters, and other marketing materials. I understand that I will not be entitled to any compensation, royalties, or any other consideration for the use of my child's image or likeness. I further waive any right to inspect or approve the finished product, including written copy that may be created in connection with the photographs, videos, or audio recordings. I acknowledge that I have read and understood this Liability Waiver and Photo/Video Release Form, and I voluntarily agree to its terms. I intend for this document to be legally binding and to apply to all present and future dance classes, activities, events, and performances at Rhythm City Dance by Chelsea LLC.
