Prime Time Camp Waiver

I fully agree to have my child registered in Prime Time Camp for the above stated tuition. I understand that registration fee is due at time of application and a camp spot will not be secured without it.

All applications must be approved by Prime Time Camp administration prior to acceptance.

I fully agree to make payments according to schedule set forth on the website. If all camp fees are not paid by May 1st, your child will not be permitted into camp, will not receive their camp bag and shirt and will not be picked up for camp.

Registration Fee is NON REFUNDABLE

Camp tuition is non-refundable after April 1st.

No refunds will be given for early departures or absences.

I acknowledge that my child’s medical history, allergy information, vaccination history and medical insurance information must be submitted prior to beginning camp. Campers will not be allowed to attend camp without this.

I understand that my child will be under the guidance and authority of Prime Time Camp. I agree that my child is allowed to go wherever staff decides to take the camp unless otherwise notified by a parent or guardian in advance.

I understand that accidents can happen at camp. In such a case that medical assistance or treatment is required I acknowledge that I, and not the camp, will be responsible for payment of that assistance.

I understand that my child is obligated to listen, obey and respect all counselors at Prime Time Camp. If my child is found to consistently disobey the rules of Prime Time Camp, or displays extreme behavior that endangers the safety of campers or staff, I understand that he or she may be subject to suspension or expulsion from camp without refund of camp tuition, according to the discretion of camp administration.

By signing this waiver, I hereby waive, release and discharge any and all claims of damages, personal injury and property damages which may occur to my child as a result of participation in Prime Time Camp. This release is intended to discharge in advance Prime Time Day Camp corporation, Dr. David Matalon and Robert “Bobby” Matalon, as well as all Prime Time Day Camp staff from any and all liability, even though that liability may arise out of perceived negligence on the part of the parties mentioned above.
