ROOT & ROAM ANIMAL CARE , CLIENT SERVICE AGREEMENT

Thank you for trusting Root & Roam Animal Care with your pet's health and comfort. My mission is to provide gentle, fear-free, in-home veterinary assistant services to reduce stress and make pet care easier for both pets and their people. Please review and sign this agreement before services begin.

I. Services & Scope: All services are performed in your home. I provide fear-free, gentle handling for pets and only offer services within my scope as a veterinary assistant and student veterinary nurse. I do not replace the care of a licensed veterinarian and will refer you to one if needed.

A non-refundable retainer of 25% or $20 (whichever is greater) is required to secure all appointments.

Established clients with a good payment history may pay at the time of visit.

3. In-Home Environment & Safety

A clean, safe, and hygienic environment is required for all visits.

I do not tolerate secondhand smoke or unsafe/dangerous environments.

4. If I feel an assistant is required to safely handle your pet, you agree to an adjusted service price for that visit. We will discuss ongoing adjustments before the next appointment.

5. Minimum Services Per Visit: To ensure my travel and time are used efficiently, each visit must include a minimum of two billable services, which can be for the same or multiple pets in the home.

6.You agree to fully disclose all known medical conditions, allergies, medications, and recent procedures for your pet(s

 

 

7. I reserve the right to refuse or discontinue services if safety, welfare, or compliance concerns arise.

8. Late Arrival: If the client is not ready within 10 minutes of the scheduled appointment time, the appointment may be canceled or rescheduled with a fee.

9. Referral & Rewards Program

I truly value your trust and support. To show my appreciation:

Referral Reward. If you refer a new client who books and completes a paid visit, you will receive $5 off your next service.

Loyalty Reward - After every 6 completed visits, you may choose one free service: nail trim, ear cleaning, or medication administration (oral/topical/injectable) Rewards are non-transferable and must be redeemed within 6 months of earning.
