Audit Change Request revised 8-2025

Use this form to request changes if you need to rearrange your MHS-eligible coursework to help satisfy your MHS requirements.

Requested changes will be approved by your MHS advisor and then sent on to the dean’s office and records office.

Your MHS advisor can only request changes to the MHS portion of your audit. If you need to make changes to another major or minor in your audit, contact the appropriate person in that program.

You can see all your courses that are eligible for MHS credit by looking under "MHS GPA" in your degree audit.

If your request involves seeking MHS elective or concentration credit for a course that is not already eligible for MHS, use the Course Variance Request Form instead.
