    Before your child’s first day, please review and sign the required GPA PMA agreements: 1) Fee Agreement 2) PMA Agreement 3) Participation Release 4) Dismissal Acknowledgement 5) Photography Release. These agreements are an important part of your membership in GPA PMA and ensure clarity for families and staff throughout the year.
  • Section 1 – Tutoring Program Fee Agreement

  • By signing below, I         (Print Name) agree, that I am a parent or legal guardian of the student named below and that I will provide a monthly payment to GPA PMA via ZELLE account @516-835-9817 or davidmachson1@gmail.com (David) who will in turn pay for tutoring services and the facility rental fee. I understand that it is my responsibility to provide full monthly payments on time for facility rent, tutoring and provided services or membership will be revoked. If there is a family hardship, contact: GPAFamilies123@gmail.com.

    The GPA PMA center runs from September 2025 to June 2026 or the end term determined by GPA PMA. It is understood that I am required to give 60 day notice (on the first of any given month) to the Board in writing. If a member terminates the agreement prior to the June 2025 end date, upon notice, the regular monthly invoice is due and the prepaid month is credited toward your final 30 days.

    It is understood that full payments must be submitted by the 1st of every month and received no later than 5 days after. Payment not received by the 5th of each month will be considered late and a $50.00 late fee will be applied to your monthly payment.
    Payment Dates: September 1, 2025, October 1, 2025, November 1, 2025, December 1, 2025, January 1, 2026, February 1, 2026, March 1, 2026, April 1, 2026, May 1, 2026, June 1, 2026.

  • Section 2 – Private Membership Association Agreement

    By joining GPA PMA, a Private Membership Association (herein defined as the ‘Association’) and/or any Facebook Group, or any other Social Media Group started by, created by, or organized by the Association, I agree to the terms and conditions of GPA PMA as follows:

    1. This Association of members declares that our objective is to allow the Private Membership Association founder, officers and all Private Membership Association members with a platform in which to conduct all manner of private business with the Association and with other Associations and Association members, keeping all business in the private domain and utilizing the protections guaranteed by the Constitution to conduct business in private and to provide a platform for members to conduct business in the private domain under all protections acknowledged and guaranteed by the Constitution. We believe that the Constitution of the United States of America and the Charter of Rights of Canada
    2. We believe that the Constitution of the United States of America and the Charter of Rights of Canada guarantees our members the rights of free speech, petition, assembly, and the right to gather together for the lawful purpose of helping one another in asserting our rights protected by those Constitutions, Charter and Statutes, in addition to the rights to be free from unreasonable search and seizure, the right to not incriminate ourselves, and the right to freely exercise all other unalienable rights as granted by God and guaranteed by those Constitutions, Charter, and Statutes.
    3. We declare the basic right of all of our members to decide for themselves which members could be expected to give wise counsel and advice concerning all matters, including homeschool education and assistance, law, and any other matter and to accept from those members any and all counsel, advice, tips, whom we feel are able to properly advise and assist us.
    4. We expect the freedom to choose and perform for ourselves the types of services and enrichment opportunities that we think best for our members, and their children for achieving and maintaining homeschool opportunities of enrichment and education, as well as the freedom to choose for ourselves any types of assistance which may be made regarding education, law and any other business activity.
    5. The mission of this Association is to provide members with a forum to conduct private contractual business between members in the private domain with the protections guaranteed within the aforesaid Constitution and Charter remaining fully intact.
    6. The Association will recognize any person (irrespective of race, color, or religion) who have joined this Association or any social media group organized, created, or managed by the GPA PMA and is in agreement with these principles and policies as a member of this Association, providing said person has not been sanctioned or banned by the president/administrator, and will provide a medium through which its individual members may associate for actuating and bringing to fruition the purposes heretofore declared.
    7. Membership to this Association, "GPA PMA", and any of its groups may be terminated by an Association Founder or Officer vote at any time should they conclude that a specific member is not maintaining a good standing or interacting with any other members in a way that is contrary, or detrimental to the focus and principles of this Association.
    8. I understand that, since The Association is protected by the First and Fourteenth Amendments to the U.S. Constitution, it is outside the jurisdiction and authority of Federal and State Agencies and Authorities concerning any and all complaints or grievances against The Association members or other staff persons. All rights of complaints or grievances will be settled by an Association designee, committee, or tribunal and will be waived by the member for the benefit of The Association and its members.
    9. I agree to join GPA PMA, a private membership association under common law, whose members seek to help each other achieve better health and good quality of life.
    10. I enter into this agreement of my own free will without any pressure or coercion. I affirm that I do not represent any Local, State or Federal agency whose purpose is to regulate and approve products, or to carry out any mission of enforcement, entrapment or investigation. I have read and understood this document, and my questions have been answered fully to my satisfaction. I understand that I can withdraw from this agreement and terminate my membership in this association at any time, and that my membership can and will be revoked if I engage in abusive, violent, menacing, destructive or harassing behavior towards any other member of The Association.   • Section 3: Membership Agreement / Waiver Form

  • Overview
    I do hereby apply for membership in the GPA PMA (herein defined as the ‘Association’), a private membership association, which is based on a Faith-based inspired enrichment and educational support group for homeschoolers. With the signing of this membership agreement, I accept the offer made to become a member of the GPA PMA and have read and agree with the following Declaration of Purpose, Preamble and Memorandum of Understanding.
    Declaration of Purpose:
    The purpose of GPA PMA is to provide a safe and nurturing environment for the young child to grow and to thrive. Through a daily rhythm of play, movement, songs and stories, GPA PMA will foster the imagination of the young child. GPA will provide elementary, intermediate, and secondary enrichment for homeschoolers.
    GPA PMA will provide and share parental guidance through a Faith-based inspired and developmentally appropriate approach for the young child. GPA PMA founders and GPA PMA Members (including parents) can freely share, discuss, and exchange information and services by members of the GPA PMA while keeping all business within the private domain.
    Membership
    By consenting to the Membership Agreement provided, members are granted the following rights and benefits:
    -Right to detailed informed consent about the services rendered, and what to expect
    Upon request, access to the Association’s Bylaws
    -Guarantee that all personal information of the Member will remain within the GPA PMA
    -Membership with The GPA PMA will remain intact unless the member submits in writing their desire to withdraw membership or membership is terminated by The Association in accordance with bylaws
    -Complaints against a The GPA PMA member must be submitted in writing to The GPA PMA within three months of the incident of complaint. These will be sent to the Person, Committee, or Tribunal designated by The GPA PMA for evaluation and remediation action.
    Preamble
    Members in the GPA PMA choose to operate pursuant to the Constitution of the United States of America, the Constitutions of the various States of the United States of America, and the Universal Declaration of Human Rights which guarantees our members the rights of free speech, petition, assembly, and the right to gather together for the lawful purpose of advising and helping one another in asserting our rights under law.
    IT IS HEREBY DECLARED that we are exercising our right of “freedom of association” as guaranteed by the 1st and 14th Amendments of the U.S. Constitution and equivalent provisions of the various State Constitutions. This means that our association activities are restricted to the private domain only.
    We proclaim the freedom to choose for ourselves the type of education philosophy that we think best. We encourage our members to perform their own research by studying different educational philosophies for the development of the child.
    Specifically, the mission of GPA PMA is to provide a safe and nurturing environment for the child to grow and to thrive. Through principles inspired by Faith-based enrichment and education, GPA PMA will have a daily rhythm appropriate to the child’s age and development that may include field trips, movement, music, art, health, reading and writing, arithmetic, history and/or science for homeschool children in elementary, intermediate and secondary grades. GPA PMA will utilize Certified tutors, teachers, parent volunteers and other experienced professionals to provide a rich Faith-based inspired learning experience. GPA PMA will recognize any person (irrespective of age, race, creed, gender, sexual orientation or religion) who has been approved as a member and acts in accordance with these principles and policies as a member, and will provide a medium through which its individual members may associate for actuating and bringing to fruition the purposes.
    Memorandum of Understanding
    I,       understand that members and service providers within The GPA PMA are or may not be licensed educators, teachers, or coaches.
    I understand that within The GPA PMA a private contractual relationship exists as a contract member-to-member association relationship. I have freely chosen to become a private member of The GPA PMA Education heretofore declared Association and thereby change my legal status as a public person, customer, or client to a private member of The GPA PMA.
    I understand that, since The Association is protected by the First and Fourteenth Amendments to the U.S. Constitution, it is outside the jurisdiction and authority of Federal and State Agencies and Authorities concerning any and all complaints or grievances against The Association, any Trustee(s), members or other staff persons. All rights of complaints or grievances will be settled by an Association designee, committee, or tribunal and will be waived by the member for the benefit of The Association and its members. By agreeing to this membership form I agree that I have sought sufficient education to determine that this is the course of action I want to take for myself and my children.
    Because the privacy and security of membership records maintained within The Association, which are held to be inviolate by the U.S. Supreme Court, the undersigned member waives HIPAA privacy rights and complaint process. However, any medical or healthcare records kept by members of The Association will be strictly protected and only released upon written request of myself as a member. Medical information will only be released if there is a medical emergency.
    I agree that violation of any waivers in this membership contract will result in a no contest legal proceeding against me.
    Other aspects of informed consent may take place in my discussions with the providers and my fellow members of The Association. As I am voluntarily choosing this method of enrichment and educational support and service providers, I will not hold The Association or its members financially liable for any particular outcome.
    I agree to join the GPA PMA, a private membership association under common law, whose members seek to help each other achieve better health and good quality of life.
    My activities within The Association are a private matter that I refuse to share with the State or local Board of Education or other governmental regulatory agency without expressed specific permission. All records and documents remain as property of The Association, even if I receive a copy of them. I fully agree not to pursue any course of legal action against a fellow member of The Association, unless that member has exposed me to a clear and present danger of substantive evil, and upon the recommendation and approval of the Association.
    I enter into this agreement of my own free will without any pressure or coercion. I affirm that I do not represent any Local, State or Federal agency whose purpose is to regulate and approve products, services or practices, or to carry out any mission of enforcement, entrapment or investigation. I have read and understood this document, and my questions have been answered fully to my satisfaction. I understand that I can withdraw from this agreement and terminate my membership in this association at any time, and that my membership can and will be revoked if I engage in abusive, violent, menacing, destructive or harassing behavior towards any other member of The Association. These pages consist of the entire agreement for my membership in The Association.
    Payment of any dues, fees or program costs, if applicable, and delivery of these signed documents to an Association representative is considered sufficient for my one-time membership contract. Membership Term begins with the start of services received on September 21, 2025 and hereafter are validated by date of this contract.

  • Section 4 – Release for Participation in Event or Activity

  • In exchange for participation in Homeschool Fraternization and Activities (the “Activity”), organized by GPA PMA (“Releasee”), I hereby agree as follows:

    1. I and anyone claiming on my behalf release, indemnify, hold harmless and forever discharge Releasee and its affiliates, successors and assigns, officers, employees, representatives, partners, agents, and anyone claiming through them (collectively, the “Released Parties”), in their individual and/or corporate capacities from causes of action of any nature and kind, known or unknown, which I may have against Releasee or any Released Parties arising out of or relating to any injury, loss or damage to person and property that may be sustained as a result of participation in the Activity (“Claims”).
    2. I understand that participation in the Activity involves inherent risks, including risk of physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent paralysis and/or death, and I assume all related risks and voluntarily participate in the Activity.
    3. I agree to indemnify Releasee, David and Ramona Machson and Rental Facility named “St. Ignatius Loyola” located at 129 Broadway Ave, Hicksville, NY 11801, against any and all claims, actions, lawsuits, costs, damages and judgments, including attorney’s fees, arising out of or relating to my participation in the Activity. No action shall be brought against Releasee and St. Ignatius at any time, for any reason, under any circumstances.
    4. This Release shall not be in any way construed as an admission by the Releasee that it has acted wrongfully with respect to me or any other person, that it admits liability or responsibility at any time for any purpose, or that I have any rights whatsoever against the Releasee.
    5. This Release shall be binding upon the parties and their respective heirs, administrators, personal representatives, executors, successors and assigns. I have the authority to release the Claims and have not assigned or transferred any Claims to any other party. The provisions of this Release are severable. If any provision is held to be invalid or unenforceable, it shall not affect the validity or enforceability of any other provision. This Release constitutes the entire agreement between the parties and supersedes any prior oral or written agreements or understandings between the parties concerning the subject matter of this Release. This Release may not be altered, amended or modified, except by a written document signed by both parties. The terms of this Release shall be governed by and construed in accordance with the laws of the State/Commonwealth of New York.
    6. I have carefully read and fully understand all the provisions of this Release and am freely, knowingly, and voluntarily entering into this Release. No person or circumstance has forced me to participate in the Activity, and I wholeheartedly choose to join and participate in said activity, as does my child(ren).


  • Section 5 – GPA Dismissal Form Acknowledgment

  • GPA Members/Volunteers are expected to know and supervise the implementation of parental/guardian plans for dismissal of children from the GPA facility, located at St. Ignatius Loyola 129 Broadway Hicksville, NY 11801.
    It is the responsibility of parents/guardians to notify the GPA front desk (see volunteer sheet) as to the preferred plan for the dismissal of their children, as well as be familiar with the GPA calendar and dismissal times via the Remind App and/or electronic email.
    If this form has not been submitted to the front desk by dismissal time with any changes for pick-up, it will be understood that only approved family members will be picking up the child.
    This is very important if a child walks home, alone or with a group, or if a parent (or other adult/group) meets the child at a designated location other than GPA facility grounds on a regular basis.
    I understand that GPA volunteers/members will follow the plan indicated above every day. Changes to this plan may only be made in writing and provided in advance. In the event of an unexpected situation, I will contact the front desk volunteer of the GPA as soon as possible to advise of any different arrangement for my child’s dismissal that day. It is my responsibility to update this form as needed.
    I agree to indemnify and hold everyone harmless that is participating in child enrichment activities taking place at the St. Ignatius Loyola 129 Broadway Hicksville NY 11801 from any loss, costs, damages, fees or any circumstance that may arise as a result of my aforementioned child’s dismissal.

  • Section 6: Photo & Social Media Release Permission Slip


  • At GPA PMA, we love celebrating the everyday moments that make our community so special. For more than seven years, we’ve shared photos and short videos from our classrooms, field trips, moving up ceremonies, and other events in our yearbook, newsletters, around our building, and during open houses.

    We also use the Band app for classroom communication, where teachers may share photos and updates with families in their class. These images help capture the joy, learning, and friendships that are at the heart of GPA PMA, and they allow us to share that spirit with our families—past, present, and future. By indicating your preference below, you help us plan ahead—especially for our first day memory photos.
