  • eGFR Weight Prescribe the correct dose
    >10 <50kg Dalteparin 2500 units S/C once daily
  • eGFR Weight Prescribe the correct dose
    >10 50-100kg Dalteparin 5000 units S/C once daily. For eGFR 11-30, monitor anti-Xa levels after 10 days as per LUHD guidelines.
  • eGFR Weight Prescribe the correct dose
    >10 101-150kg Dalteparin 5000 units S/C twice daily (Off-license dose). For eGFR 11-30, monitor anti-Xa levels after 10 days as per LUHD guidelines.
  • eGFR Weight Prescribe the correct dose
    >30 >150kg Dalteparin 7500 units S/C twice daily (Off-license dose)
  • eGFR Weight Prescribe the correct dose
    <10 >150kg Please consult Haematology for advice
  • eGFR Weight Prescribe the correct dose
    ≤10 or on renal replacement Any Consider mechanical measures. If high thrombotic risk consider dalteparin 2500 units once daily
  • DO NOT PRESCRIBE DALTEPARIN - YES TO ONE OR MORE CONTRAINDICATIONS.

    PLEASE RE-CONDUCT THIS REVIEW EVERY 24 HOURS OR WHEN THE PATIENT'S CLINICAL CONDITION CHANGES.
  • Consider TED stockings.

    CONTRAINDICATIONS TO TEDS
    • Severe peripheral arterial disease
    • Leg and/or foot ulcers
    • Major limb deformity
    • Cellulitis or dermatitis
    • Peripheral neuropathy of legs
    • Fragile skin or skin allergy to TEDS
    • Severe leg oedema

     

  • DO NOT PRESCRIBE DALTEPARIN – NO SIGNIFICANT VTE RISK FACTORS.
  • Consider TED stockings.

    CONTRAINDICATIONS TO TEDS

    • Peripheral neuropathy of legs
