    The Delray Beach Community Redevelopment Agency (CRA) is currently seeking tenants who provide affordable medical, healthcare, pharmacy, and wellness services in its newest building located in downtown Delray Beach - one block from Atlantic Avenue. 

    The Rental Space is located at 95 SW 5th Avenue, Delray Beach and consists of three (3) separate suites that are ready for build-out.

    The CRA-owned commercial Rental Space is within the West Atlantic Avenue Corridor, prominently positioned at corner of the Historic SW 5th Avenue within the The Set neighborhood — an area that was once a vital hub for bustling Black-owned businesses that served the residents in the surrounding community.

    The Rental Space is just minutes from I-95 and approximately thirty minutes from Fort Lauderdale International Airport and Palm Beach International Airport.  The Rental Space is within walking distance of shops and restaurants in downtown Delray Beach and two miles from the beach. Most importantly, the Rental Space located where the commercial corridor transitions into an established residential community. 

    In 2017, The Set community brand was identified by the long-standing residential and business communities surrounding the West Atlantic Avenue corridor. The Set community encompasses the area that is bounded by I-95 to the west,  Swinton Avenue to the east, Lake Ida Road to the north, and SW 10th Street to the south.

    The Set is predominately a single-family residential community that includes Delray Beach’s historic main streets of Atlantic and Fifth Avenues traversing the east-west and north-south corridors of the western end of downtown Delray Beach.

    • One Ground Floor Suite - Approximately 1,075 square feet
    • Two Second Floor Suites - Approximately 1,228 and 2,504 square feet

    • Approximate monthly rental rate (not including utilities, internet, phone, security, interior pest control):
      • Rental Rate to be negotitated with each tenant
      • Negotiated Rental Rate will increase 6% annually
      • Rental Term: Ten (10) years with options to renew
    • All three (3) suites are ready for tenant build- out and may all be leased by one (1) tenant or may be leased to three (3) separate tenants.

  • For this Rental Space, the Delray Beach CRA invites all interested medical, healthcare, pharmacy, and/or wellness service providers to submit their proposals to offer affordable medical, healthcare, pharmacy, and/or wellness services at 95 SW 5th Avenue,  for consideration as tenants of this prime location on the historic SW 5th Avenue commercial corridor in downtown Delray Beach. 

    The CRA is committed to curating a diverse and interesting mix of business offerings along historic NW/SW 5th Avenue that meets the needs of the surrounding community based on the 2012 West Atlantic Area Needs Assessment and the West Atlantic Master Plan (aka The Set Transformation Plan).

    The 2012 West Atlantic Area Needs Assessment identified the following community needs:

    - Pharmacy Services

    - Health and Wellness Services and Facilities (including but not limited to urgent care and/or clinic, primary medical, optical, dentistry, fitness center, mental health, etc.)

    - Financial Services (bank, credit union, financial planning advisors, personal banking, etc.)

    - Family and/or Social Entertainment 

    The West Atlantic Master Plan (aka The Set Transformation Plan) also identifies the need for affordable commercial spaces for small, local businesses that will serve the surrounding community:

    - retail/specialty shops and services

    - professional offices (CPA, legal services, insurance, etc.)

    - artist gallery/workspaces

     

    The Delray Beach CRA reserves the sole and absolute right to select tenants that best serve the interests of the CRA District.

    Any interested business will also need to be permitted within the Central Business District zoning district, more particularly described in the City’s Land Development Regulations.

  • ELIGIBILITY

  • Applicant criteria:

    • Applicant must be a medical, healthcare, pharmacy, or wellness service provider that offers affordable medical, healthcare, pharmacy, and/or wellness services that may be used by residents of The Set.
    • Applicant must accept a range of health insurance types, including  Medicare and Medicaid. 
    • Applicant must hold an active State of Florida license necessary to provide the proposed medical, healthcare, pharmacy, and/or wellness services at the Rental Space.
    • Applicant must demonstrate that they have a minimum of two (2) years' of experience as of the due date of this Application managing and operating a medical, healthcare, pharmacy, and/or wellness office that accepts a range of health insurance types, or two (2) years' of equivalent, relevant experience providing medical, healthcare, pharmacy, and/or wellness services, and performing the equivalent, relevant duties and responsibilities required to operate a medical, healthcare, pharmacy, and/or wellness office that accepts a range of health insurance types.
    • Applicant can only submit one application.
  • APPLICATION PROCESS

  • Applications will not be considered until all required documentation is submitted.

    Complete application packets must include the following documentation:

    a. Completed online application submitted by the deadline. Late submissions will not be considered. 

    b. Complete Business Overview and Plan - provide all details of the business including:

    • Overview of the medical, healthcare, pharmacy, or wellness practice including the patient population to be served at the Rental Space location. 
    • All medical, healthcare, pharmacy, or wellness providers to be located at the Rental Space, including education, experience, specialities, certifications, etc.
    • All medical, healthcare, pharmacy, or wellness  services to be offered at the Rental Space.
    • All insurance plans to be accepted at the Rental Space, to include Medicare and Medicaid.
    • Hours of Operation for the Rental Space location.
    • Plans for anticipated buildout of the Rental Space and launch timeline. Timeline should account for any site plan and permit approval. 

    c. Description of the legal organizational structure of the Applicant entity including the title and names of the officers/partners/directors/members over the last two (2) years (if the applicant entity is affiliated with another entity, please provide a statement representing the nature of the affiliation along with the other entity’s name, address, phone number, and a listing of the officers/partners/directors/members).

    d. Three-year financial projections of business operations/maintenance.

    e. Budget for business operations, including startup costs.

    f. Evidence of financial ability/capital to operate (may include approved loan, cash balance, line of credit, etc.)

    g. IRS Income Tax Returns for the business (must match applicant name), most recent 2 years' worth. 

    h. Current Business Tax Receipt (provide a copy of the business's current business tax receipt for the current location of operation, if available at the time of submission. If not available, the Successful Tenant will need to provide a City of Delray Beach Business Tax Receipt before any agreement can be finalized).

    i. Business License & Registration with Florida Department of Business and Professional Regulation (DBPR)

    j. Professional Licenses and Certifications: provide copies of all current, up to date state licenses, board certifcations, and other certifications as applicable for the medical, healthcare, pharmacy, and/or wellness service providers who will be located at the Rental Space. 

    k. Health Insurance Acceptance - provide proof that Applicant has a National Provider Identifier Number, that the Applicant will be able to successfully complete  or has previously successfully completed the credentialing and contracting process (for a different location owned and operated by the Applicant) with health insurance companies, and government programs -Medicare and Medicaid. 

    m. Malpractice Insurance or exemption information. 

    l. Other pertinent information.

    Any information submitted after the application deadline will not be considered.

  • REVIEW & SELECTION PROCESS

  • All applications will be reviewed and scored by the Selection Committee.  Final decisions will be made by the Selection Committee.

     

    Review Process:

    • Preliminary review of application submissions to ensure completion and eligibility. 
    • The responsibility for submitting a complete application on or before the stated time and date is solely and strictly that of the Applicant. Late and incomplete applications will not be accepted.
      • An Applicant may submit a modified Application to replace all or any portion of a previously submitted Application or withdraw an Application at any time prior to the Submission Due Date. All modifications or withdrawals shall be made in writing, via email. Oral/Verbal modifications are prohibited, and they will be disregarded. Written modifications will not be accepted after the Submission Due Date. The CRA will only consider the latest version of the Application.
    • Selection Committee review and score eligible applications and related materials.
    • Notify applicants
    • Execute Lease Agreements 

     

    Selection Criteria:

    Applicants will be scored on the following:

    Section I:  Organization Capacity & Qualifications (20%)
    Section II: Experience and track record operating similar business/program/project (25%)
    Section III: Strength of the Application/Proposal Narrative and Business Plan (20%)
    Section VI: Budget, Finances, Sustainability (25%)
    Section V: Overall Proposal (10%)
