I, the below named individual, acknowledge that participation in athletic events necessarily involves risk of physical injury. I further acknowledge the recreational programs held in the Family Life Center of Christ United Methodist Church or anywhere on Christ United Methodist Church premises are primarily administered by people who volunteer their time, rather than paid professionals. In consideration for permitting the voluntary participation of said individuals in its programs, I hereby release, discharge, and hold harmless Christ United Methodist Church and its employees, volunteers, and other representatives from any and all claims for injuries, damages, or losses arising from participation that may result to me, or anyone under 18 for which I am signing on their behalf, while participating in any Christ United Methodist Church sponsored event or in the event I am allowed by a church official to use the recreational facilities of this church for my private use. Youth under 18 years old need parental permission. This waiver includes any and all claims, including claims of negligence.
