The Rockin' Chair – Photo Submission & Release Form

I, Full Name*, hereby grant The Rockin’ Chair (“Company”) the irrevocable, worldwide, perpetual, and royalty-free right to use, reproduce, edit, publish, distribute, and display any photograph(s) and/or video(s) (“Images”) in which I appear, in whole or in part, in any form and in any media, now known or later developed, for marketing, promotional, educational, or other lawful purposes.
I confirm that:

• If I am the photographer, I am the sole copyright owner of the Images and grant the Company full permission to use them as described above, without additional payment.
• If I am not the photographer, I have obtained the photographer’s written permission to share the Images and authorize the Company to use them.

I waive any right to inspect or approve the finished Images or any materials in which they appear and understand I will not receive additional compensation. I release and hold harmless The Rockin’ Chair, its owners, employees, agents, and assigns from any and all claims, demands, and causes of action arising out of or in connection with the use of the Images. This agreement is governed by the laws of the state in which the Company is headquartered.
