    At KO-NEKT, we welcome all feedback — whether it’s a compliment, a comment, a complaint, or a whistleblowing concern. Your views help us improve our services, celebrate successes, and address any serious issues.

    Please use this form to share your experience or raise a concern. Your information will be handled confidentially and fairly.

    • Wherever possible, we encourage complaints to be resolved informally by speaking directly to the staff member involved or their supervisor. If you wish to make a formal complaint, please complete this form in full. Formal complaints should include the nature of the complaint, the date(s) and time(s) of the incident(s), the names of those involved (if applicable), and your desired resolution, if any. Once received, your complaint will be acknowledged within 5 working days. It will be assigned to a designated investigator who will conduct a thorough and impartial investigation, gather relevant information and evidence, and interview those involved if necessary. A response will be provided within 15 working days of acknowledgment. If more time is needed, we will inform you of the reason for the delay and provide an updated timeline. Outcomes may include an explanation and/or apology, changes to policies or procedures, staff training, or disciplinary action. If you are not satisfied with the outcome, you may appeal by completing a new form marked “Appeal” within 10 working days of receiving the outcome. The appeal will be reviewed within 20 working days, and a final decision will be communicated to you.

      Compliments can be submitted through this form and should include details of the service or staff member being complimented along with a description of what was done well. All compliments will be recorded and shared with relevant staff and management to recognise and celebrate achievements.

      Comments can also be submitted through this form. All comments will be reviewed by management, and suggestions will be considered during regular service evaluations. Where appropriate, feedback on the implementation of suggestions will be provided.

      All feedback will be handled in line with KO-NEKT’s confidentiality and data protection policies. Personal information will only be shared as necessary to resolve complaints or act on feedback. This policy will be reviewed annually to ensure it remains effective and aligned with best practices, and feedback trends will be analysed quarterly to identify patterns and areas for improvement.

      At KO-NEKT (Childcare) CIC, we are committed to the highest standards of integrity and ethical conduct. We want to make it easy and safe for anyone — including staff, volunteers, service users, contractors, and stakeholders — to report concerns about misconduct, malpractice, or unethical behaviour.

       

      Purpose of this section
      This part of the form allows you to report a whistleblowing concern confidentially. All reports will be taken seriously, investigated thoroughly, and acted upon appropriately.

      What to report
      You can use this form to report:

      • Fraud or financial misconduct
      • Health and safety violations
      • Abuse, neglect, or exploitation
      • Discrimination, harassment, or bullying
      • Breaches of company policy or the law
      • Any other serious wrongdoing or unethical behaviour


      Confidentiality and protection
      Your identity will be kept confidential as far as possible, consistent with the need for a fair investigation. KO-NEKT prohibits retaliation or adverse treatment against anyone who raises a concern in good faith.

       

      How it will be handled
      Once submitted, your concern will be reviewed and assigned to an appropriate investigator. We will keep you informed of progress and the outcome where possible. If wrongdoing is confirmed, corrective or disciplinary action will be taken.

       

      Important note
      We take false or malicious reporting seriously. Anyone found to have knowingly made a false report may face disciplinary or other appropriate action.

      This whistleblowing procedure is reviewed annually to ensure it remains effective and compliant with relevant laws.
