By checking the box below you are indicating your understanding and acceptance of the rules, guidelines, and expectations set forth by the course provider for participation in the class. This agreement signifies a commitment to abide by those terms throughout the duration of the course.
By checking the box below you are indicating you accept and agree to make payments to Ezer Biblical Counseling Center (EBCC); for the sum of $351 in installments, of $117 every month of the duration of the cohort group class (Dark Night of the Soul chohort group).
The first installment must be received by EBCC upon signup for this course. Every following instalment must be recieved by EBCC no later than 3:00pm the first day of each month of the duration of the three month course (February 1 - April 1).
Should two instalments not be paid as agreed, EBCC may, without any further notice, cancel this agreement and refer my outstanding charges to an external agency. (Please note this action may be taken for outstanding charges only. This does not include voluntary payments or contributions).
I agree to notify EBCC immediately if I change contact details. I understand that, if I do not notify EBCC of such a change, EBCC may, without further notice cancel this agreement and refer my oustanding charges to an exteral agency.
In the event of myself leaving EBCC, I understand that this agreement will still apply until the amount of the debt is paid.
This agreement will only be operative once I check mark the agree terms & conditions of this form.
This agreement is approved by the owner of Ezer Biblical Counseling Center (EBCC).
