All For Recovery Photo Release Form

I hereby grant All For Recovery, its representatives, employees, and agents the irrevocable right to take and use photographs and/or video recordings of me, and to use these in any and all media, now or hereafter known, for the purpose of promoting, publicizing, or documenting the activities of the organization.

I understand that these images may be used in printed publications, on the organization’s website, social media channels, in promotional materials, or for other lawful nonprofit-related purposes. I waive any right to inspect or approve the finished product wherein my likeness appears, and I waive any right to royalties or other compensation arising from or related to the use of the photographs or recordings.
