Welcome to the Small Business Revitalization Grant Program Application

Thank you for your interest in the Small Business Revitalization Grant Program. This program is designed to support eligible Georgia-based, for-profit small businesses that were negatively impacted by COVID-19 and have completed the Money Smart Business Program through one of Georgia’s 12 Regional Black Chambers. Before starting your application, please carefully review the eligibility requirements, required documentation, and allowable use of funds listed below. Having all materials ready will ensure a smooth and timely submission process:
- business is located in Georgia
- business is for-profit
- fewer than 500 employees
- business began operating on or before March 3, 2020
- business experienced a negative economic impact from COVID-19
- business Participated in the Money Smart Business Program hosted by one of the 12 Regional Black Chambers in GA
Documents
- Valid photo ID
- Current Certificate of Existence
- 2019, 2020, and 2021 business tax returns
- Obtain tax transcripts from IRS (online) for the tax years 2020 and 2021
- Three most recent business bank account statements
- Completed and signed W-9 form
Use of Funds

I will use the grant for eligible business expenses, such as:
- Payroll
- Rent
- Utilities
- Other operational costs
Submitting this application does not guarantee funding. All applications will be reviewed for completeness and eligibility. Incomplete applications or those missing required documentation will not be considered. If your application is approved, you will be notified by email with next steps.
