Our office requires a minimum of 48 hours’ notice for any appointment cancellation or rescheduling. This allows us to offer the time slot to another patient who may be on a waiting list. We block several hours for each appointment to ensure you receive the care and attention you deserve without any interruption or delay.
If you cancel an appointment with less than 48 hours’ notice, or if you do not show up to the appointment, you will be charged a flat fee of $250. We will make every effort to reschedule your appointment to avoid your being billed this fee. This fee would not be covered by your insurance.
This policy applies to all appointments, including initial consultation, assessment/testing, and feedback.
By signing below, I acknowledge that I have read, understand, and agree to the cancellation and no-show policy detailed above. I understand Dr. Kalinian’s staff will bill me the amount of $250 in the event of a late cancellation or no-show. Medical emergencies/hospitalizations are excluded from this policy. We make every effort to reschedule your appointment to avoid charging this fee and will notify you that a late cancellation/no show fee will be added to your account balance.
