1. ALL FIELDS ARE REQUIRED TO BE FILLED; if not applicable, comment N/A.
2. There are two options for completing this form;
i. Owner/Pilot Checks: checks on recreational gliders may be carried out by the OWNER/PILOT, with minimum HG NOV certificate, under supervision of an authorised WOF Inspector.
ii. Authorised WOF Inspector Checks: checks can be done soley by the Authorised WOF Inspector;
3. If you are an Authorised WOF inspector assessing your OWN glider, select Authorised WOF inspector;
4. Minimum standards are the manufacturer's specifications, or if none, are to the Inspecting Officer's best informed knowledge;
5. Dents in the carbon D-sections and aluminium tubes must be individually assessed, in accordance with the manufacturer's guidelines, or through consultation with an experienced inspector, to determine if structural integrity is affected, before deciding on repair or replacement;
6. Repaired parts must be at least as strong as the original and must not affect the glider's trim or put extra stress on other areas;
7. Generally, inspection is VISUAL only. Removal of the sail for inspection is recommended;
8. Non-factory modifications made to the glider, and judged acceptable, must be noted in the COMMENTS BOX;
9. Document modifications deemed unacceptable and/or repairs required and/or items that may soon require attention IDENTIFY in the COMMENTS BOX;
10. If the glider has PASSED inspection, fill out all fields on a current NZHGPA WOF label and stick the label on the glider keel, aft of the trailing edge and in plain sight;
