Informed Consent and Acknowledgment 

I, the undersigned, hereby acknowledge and agree that participation in football training sessions, games, and activities organised by Olsen Football involves certain inherent risks. These risks may include, but are not limited to, physical injury, collisions with other participants, impacts from equipment, slips and falls, environmental conditions, and other unforeseen hazards.

 
I confirm that the player named in this form is physically fit and able to participate in football training and that I have disclosed all relevant medical information to Olsen Football prior to participation.

I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, which might be sustained as a result of participation in Olsen Football activities, except where prohibited by law.

 
I release, waive, discharge, and hold harmless Olsen Football, its owners, directors, coaches, staff, agents and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during training or related activities.

Medical Consent

 
In the event of an injury or medical emergency, I authorise Olsen Football staff to provide first aid treatment and, if necessary, seek medical assistance on behalf of the player. I accept responsibility for all costs associated with such treatment and transportation.

I acknowledge that I have read this document in its entirety and understand the above. I have had the opportunity to ask questions and receive answers.

Code of Conduct
I agree that the player will:

Show respect to coaches, staff, teammates, opponents, and match officials.
Refrain from disruptive, aggressive, or unsportsmanlike behaviour.
Follow all safety instructions and arrive prepared for each session.

 
Failure to adhere to this code of conduct may result in suspension or termination of participation without refund.

Media Release

I understand that photographs and/or video recordings of training sessions may be taken and used by Olsen Football for promotional purposes including, but not limited to, social media, websites, and marketing materials. I consent to such use unless I have indicated otherwise on this form.
