  • About this survey

    This survey is for alumni of YOTO programs from 1986-2020 only. We are using the survey responses to assess the long-term impact of YOTO's programming on alumni. 

    How much time is needed to participate?

    The survey will take approximately 15-20 minutes to complete.

    What happens to the information you provide?

    Your name and date of birth are used to verify that you received services from YOTO, but all data will be kept confidential and responses grouped together, so that no personal information is identifiable. 

    Agreement to participate

    Please read the following statements:

    • I confirm I participated in YOTO programs between the years of 1986-2020.
    • I understand that my responses to the survey will be securely stored and used by System Change Partners and YOTO for research and evaluation purposes only.
    • I understand the information I provide about myself is confidential. My identity will not be disclosed for commercial use by a 3rd party or made public without my explicit consent.
    • I understand that my participation is voluntary and that I can withdraw at any time.
    • I agree I have received adequate information about my participation in the survey and understand what will happen to the information I provide.

    Please provide information about yourself and your household. Your name and date of birth will be used to verify that you received services from YOTO, but will not be used to identify you in the evaluation without your explicit consent.

  • ℹ️

    • Transgender woman: A person who was assigned male at birth and identifies and lives as a woman.
    • Transgender man: A person who was assigned female at birth and identifies and lives as a man.
    • Cisgender woman: A person who was assigned female at birth and continues to identify and live as a woman.
    • Cisgender man: A person who was assigned male at birth and continues to identify and live as a man.
    • Non-binary: A person whose gender identity does not fit within the traditional categories of strictly “woman” or “man.”
    • Genderqueer: A person who identifies outside or beyond the binary ideas of woman and man, often embracing fluid, flexible, or nonconforming expressions of gender.
    • Agender: A person who does not identify with any gender, or who feels they do not have a gender.
    • Genderfluid: A person whose gender identity changes or shifts over time or in different contexts.
    • Two-Spirit: A term used within some Indigenous cultures to describe a person who embodies both masculine and feminine spirits. It is specific to Native/Indigenous traditions and should be used respectfully in those cultural contexts.
    • Other: An option for individuals whose gender identity is not listed, giving space for self-definition.
    • Prefer not to say: An option for individuals who choose not to disclose their gender identity, affirming privacy and personal choice.
