ASSUMPTION OF RISK, RELEASE, AND WAIVER
I,{guardiansName}, understand that there are risk involved in my participation in this camp and/or activity, including the risk of property damage, personal injury or death. I understand that Rain Down Ministries of Alabama and its programs do not provide liability insurance, or otherwise indemnify me or anyone else who may participate in this camp, for any injuries or any other liabilities arising from my participation.
Therefore, in consideration of my participation, I assume all risks and responsibilities surrounding this camp. I release, agree to defend, hold harmless and indemnify Rain Down Ministries of Alabama, and their officers, agents, employees, or representatives from and against all liabilities, claims, demands, or causes of actions, including claims for property damages, personal injury, or death caused by the passive or active negligence of myself and/or Rain Down Ministries of Alabama or its officers, agents, employees, or representatives, for any hidden, latent or obvious defects in equipment, or caused by any other activities of mine, or anyone else who may be a participant, during this camp.
I, {guardiansName}, understand that my participation in this camp and photo likeness may be selected for use in reporting, program materials, and outreach. In this event, confidentiality will only be maintained, and my name will not be disclosed, if I submit a written request to the office of Rain Down Ministries of Alabama. I hereby give my permission for the release of my photo and likeness thereof as it pertains to this camp.
