2025-2026 Online Questionnaire and

1. Submissions must be received/time-stamped by 5:00 p.m., September 26, 2025.

2. If you have previously submitted an application and included supporting documentation at that time, we ask that you fill out and submit the application for this year along with your current fiscal year’s budget, and the budget for which you are requesting funding (if for a different fiscal year). If necessary, we will request additional supporting documentation.

3. If you are a new applicant, please fill out the form, include the documentation requested (you can upload to this form), and submit your application.

4. Grant requests are reviewed/awarded from October 2025 through June 2026, thus it could be several months before a request is reviewed/awarded.
