    Play It Forward is a therapeutic sports program designed to use recreational sports to teach emotional regulation, teamwork, self-control, and confidence. Activities are adapted to meet the needs and abilities of each participant, focusing on building both physical and social skills in a supportive environment. It is facilitated by licensed therapists, certified peer support specialists, and approved volunteers. You will be expected to commit to our program for three months. This group meets on Tuesday and Thursday evenings 4:30-6PM.
  • 1. Program Overview
    Play It Forward is a therapeutic sports program designed to use recreational sports to teach emotional regulation, teamwork, self-control, and confidence. Activities are adapted to meet the needs and abilities of each participant, focusing on building both physical and social skills in a supportive environment.

    By enrolling in this program, you are agreeing to receive therapy, case management, and peer support services as part of your participation.


    2. Schedule & Location
    The program meets on scheduled days and times as assigned by ACTION, Inc.
    All regular sessions are held at 207 College Street, Bowling Green, KY 42014 unless otherwise specified.
    Locations may vary for special activities (e.g., outdoor field trips); families will be notified in advance of any changes.

    3. Eligibility & Enrollment
    Participants must be covered by Medicaid unless otherwise approved for a non-Medicaid opening.
    All Medicaid participants must have prior authorizations approved before starting services.
    Non-Medicaid spaces are limited and may require an out-of-pocket payment arrangement.
    Participation requires the ability to follow group instructions and engage in activities safely.

    4. Attendance Policy
    Commitment to attending both days per week is preferred for best results.
    Three missed classes will result in the forfeiture of your spot in the program. You will be able to return to the waitlist after this occurrence, and readmission will be considered based on space and program needs.
    Consistent attendance is necessary for skill development and group continuity.
    Please notify staff in advance of absences.

    5. Participant Expectations
    Demonstrate sportsmanship, respect for peers and staff, and safe play at all times.
    Follow instructions and safety rules for each sport or activity.
    Participate to the best of their ability, with modifications as needed.

    Refrain from physical and verbal aggression toward other students and staff members.

    6. Parent/Guardian Responsibilities
    Ensure timely arrival and pick-up.
    Encourage sportsmanship and positive skill use outside the program.
    Communicate any physical limitations, injuries, or health conditions to staff before each session. Talk to your child about communicating any injuries or health issues, immediately, that occur during the group.
    Provide any necessary medications before arrival, as ACTION, Inc. staff do not dispense or administer medications.

    7. Safety & Attire
    Closed-toe athletic shoes are required; appropriate sports clothing is recommended.
    Protective gear (if required for a specific sport) will be provided by ACTION, Inc. unless otherwise notified.
    Participants with medical restrictions or injuries must have clearance from a physician before participating.

    8. Food & Beverages
    Water is provided during all program hours; occasional snacks and drinks, when available, will be provided by ACTION, Inc. 
    Meals are not provided; please ensure your child eats before attending.

    9. Confidentiality
    ACTION, Inc. staff maintain confidentiality in accordance with HIPAA and applicable laws.
    While staff encourage privacy, we cannot control what other participants may share outside of the group.
