POLICIES AND PROCEDURES
INJURIES:
Parents, legal guardians of minor students, and adult students waive the right to any legal action for any injury sustained by activity at or in connection with a program or class at BRICKHOUSE NYC resulting from normal dance activity, disorderly conduct,or any other activity conducted by the students before, during, or after class time. Parents of minor dancers and adult dancers are solely responsible for any and all costs, damages, and expenses incurred by them and/or their child as a result of any injury sustained from participation in any classes taken at BRICKHOUSE NYC. BRICKHOUSE NYC requests that each student enrolled consult with a physician with respect to any past or present illness or injury that may affect their participation in and ability to endure the exercise and/or dance program. Parents and/or students agree not to hold BRICKHOUSE NYC, its staff, owners and/or instructors responsible in any way for such injuries.
PHOTO RELEASE:
Parents, legal guardians of minor students and adult students hereby grant BRICKHOUSE NYC permission to take photographs and/or videos of the students to use in brochures, social media, websites, posters, advertisements and other promotional materials. Permission is granted for BRICKHOUSE NYC to copyright such photographs and/or videos in its name.
POLICIES:
1. Dancers are granted a 15 minute grace period to participate in any session. If a dancer arrives more than 15 minutes late, they are required to sit out for that session.
2. You are required to wear at least one piece of either Kids Breaking League, X Factory, or Brickhouse NYC apparel in all training sessions.
3. Our family is expected to show kindness, support, respect, and professionalism to other crew members and staff members at all times.
4. Non-compliance with the policies of BRICKHOUSE NYC may result in the dismissal of a student. Non-compliance with the policies of Brickhouse NYC may result in not being invited back for the following season.
5. Any student found destroying or damaging BRICKHOUSE NYC property or behaving in a way inconsistent with the principles and ideals of the culture at BRICKHOUSE NYC will be promptly dismissed.
I, the undersigned, acknowledge that I have read and understand this Registration Form. I hereby authorize BRICKHOUSE NYC to charge the credit card provided for the tuition charges indicated above. I will give BRICKHOUSE NYC one month's written notice from the first of the month to discontinue these charges. I authorize BRICKHOUSE NYC to re-submit any insufficient transactions. I have read and understood these policies and procedures and I am fully aware of all the terms and principles contained herein. All questions have been answered to my satisfaction and I agree to abide by them.
