  • Please fill out this Audition Form completely. Please allow yourself about 10 to 15 minutes to complete the entire Audition Form. By submitting the information via electronic methods you are helping STAR Repertory Theatre save resources such as paper as well as allowing the production team the ability to spend less time on administrative tasks, and more time on producing a show of the highest quality.

    All information regarding our upcoming productions, camps, production fees, rehearsal dates, production dates, and parts available can be found on the STAR Repertory Theatre website at www.starrepertorytheatre.com. To ensure that we have space for you, please schedule your audition date and time as soon as possible through the Sign Up Genius link below this form. Payment of your non-refundable show production fee is due at the time of your audition.

  • If you or your child are involved in a STAR Repertory Theatre production, you are hereby advised that our company does not carry workman's comp insurance for participants or volunteers. If you should suffer an injury while participating in our production, you will be personally responsible for your medical or injury related expenses. Please enter information that can be used to contact someone in case of an emergency. Please provide several phone numbers if possible. This information will only be used to contact someone in case of an emergency.

  • Informed Consent/Acknowledgment/Medical Release and Authorization

    By participating, I hereby give my approval in any and all activities prepared by STAR Repertory Theatre during the show. In exchange for the acceptance of said candidacy by STAR Repertory Theatre, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless STAR Repertory Theatre and all its respective officers, agents, and representatives from any and all liability for injuries arising out of traveling to, participating in, or returning from selected rehearsal/show sessions.

    In case of injury, I hereby waive all claims against STAR Repertory Theatre including all directors and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the show. There is a risk of being injured that is inherent in all activities. 

    By signing below, you agree to indemnify STAR Repertory Theatre and all of their officers, their families, agents, employees, production staff, performance venues, subcontractors, independent contractors and hold them free and harmless to any property damage, bodily injury or death or any other element of damage of any kind occurring anytime you or your child is engaged in an activity or on behalf of, or related to STAR Repertory Theatre.

    As Parent and/or Guardian or participant, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of myself or a minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

    Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for myself or a minor child. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

    Permission is also granted to the STAR Repertory Theatre, and its affiliates including Directors, Assistant Directors, Choreographers, Musical Directors, and Volunteer Parents to provide the needed emergency treatment prior to myself or the minor child’s admission to the medical facility.

    This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of myself or the named minor child, in my absence.

  • Audition Information

    STAR Repertory Theatre is dedicated to giving as many parts and opportunities to students as possible. In order to do that, STAR Rep may have 2, 3, or sometimes 4 casts in one show. STAR Rep does not cut anyone from their shows, so everyone who auditions for a STAR Rep show will be cast! We invite everyone to audition, so that we may be able to place you in the best possible part! Whether a main part, or a support part, we know you will find the experience tremendously rewarding! Remember there are no small parts, just small actors!

    Students who audition should be prepared to sing a one minute song. Students should bring along a karaoke accompaniment on their cell phones...we do not have a piano player at auditions. There should not be any vocals on your accompaniment. Please do not use a song for the production you are auditioning for. The song you choose should show your personality, and your vocal abilities.

    Prepare for your audition! Do some research about the show that you are auditioning for. Get to know the characters that are featured, as well as the ensemble. Choose a song that would most closely resemble the character that you are auditioning for. The internet has a wealth of information!

  • Please list any prior show experiences including school, community theatre, church, etc. performances. You may include up to 3 shows via this web submission. Please list the list 3 performances or your highlights of major roles. Should you wish to include additional information for the artistic staff please email your resume and/or headshot to starrepertorytheatre@yahoo.com. The subject line should include Your Name/Show Name/Resume and Headshot. Our administrative staff will forward this information to the artistic team. Please bring with you to your audition a printed copy of both your resume and headshot in case the artistic team had not had a chance to review this web submission.
