BOOKING: Thank you for your interest in my makeup services! Please carefully review the attached bridal contract. I require this contract to be completed and submitted with a non-refundable deposit in order to secure your wedding date.
The complete balance for your bridal party will be due 2 weeks (14 days) before your wedding date.
Accepted forms of payment for services include: invoice, Cash App, and Venmo. Gratuity can be added to your final payment via the payment methods above, or cash on the day of the event. Cards and checks will NOT be accepted onsite. Gratuity is never expected but always appreciated, and will never be added to your invoice automatically.
TRIALS: Trials are included in each package pricing for one (1) bride only. Any additional brides or members of the bridal party requesting a trial will be priced at $150 per person.
DELAYS: A late fee of $25.00 will be charged for every 30 minutes of delay when a client is late for the scheduled time, or if the scheduled makeup application exceeds the allotted time due to client delays.
SATISFACTION GUARANTEED: Makeup will be completed to the client’s satisfaction, and acceptance of the completed makeup application is acknowledgement by the client that the makeup is done to the client’s satisfaction.
TRAVEL FEES: Travel within an hour from my salon located at 9437 Civic Center Blvd. West Chester, OH 45069 is included in my package pricing. Any additional miles will be priced at $1/minute. Travel over 1.5 hours will require overnight accomodations in addition to the travel fee. If travel over 4 hours is required, a flight and hotel accommodation will be required in additional to the minimum travel fee.
LIABILITY: All brushes, tools, and makeup products are sanitized between every makeup application and after every wedding. Makeup products used are hypoallergenic. Any allergies and/or skin conditions must be reported by the client to the makeup artist prior to application and, if need be, a sample test of makeup may be performed on the skin to test reaction.
Client(s) agree to release the makeup artist (Kelsi Garibay) from liability for any skin complications due to allergic reactions.
USE OF IMAGE: Each client will be required to take before and after pictures for insurance purposes. Clients will have the option to opt out of being posted on social media or for any advertising purposes.
PAYMENT: The final balance is due two weeks (14 days) before the event date — no exceptions. The person responsible for the entire balance of payment is the person whose name appears on this contract. The deposit paid is nonrefundable
CANCELLATION POLICY: Cancellations must be made at least thirty (30) days prior to the client’s reserved date or the client will be responsible for paying the full amount of services agreed upon in this contract. This includes dropping the number of services originally contracted.
AMENDMENTS TO SERVICES: Any additional add-ons must be made known at least 2 weeks prior. Day of add-ons are subject to artist discretion.
