This form is to be completed if you wish to request a booking for use of the Ipswich Basketball Stadium and its facilities.

(Please note casual shooting does not require a booking.)

Completing this form does not guarantee your booking. Ipswich Basketball will contact you to confirm availability or to clarify your booking request.

You will be required to pay 25% of the full cost to secure the booking.

IBA offers a full range of Canteen services daily.
- Standard Canteen - offers an extensive range of cold drinks, snacks and ice creams.
- Full Canteen - offers all standard canteen items plus a range of hot food options such as hot chips, potato scallops, dagwood dogs, dim sims, spring rolls, chicken nuggets, pizzas, etc.
- Pre-order Meal packages - packages such as nuggets, chips & a drink; pizza, chips & a drink; hotdog, chips & a drink, chicken, chips & a drink; etc. may be organised for your event.
- Catering options - IBA is more than happy to discuss catering options with you for your event. Some examples are sandwich platters; wrap platter; slider platters; hot platters (spring rolls, dim sims, garlic chicken balls, nuggets, chicken wings, etc.); cake/slice platters. Please enquire.
- Other - please feel free to discuss your specific requirements with us.

You acknowledge and agree to the following terms and conditions:
- Ipswich Basketball Association (IBA) reserves the right to cancel any booking.
- A 25% deposit must be paid within seven (7) days after IBA advises that the court is available to secure the booking. The deposit will be deducted from the total owing.
- The hirer is responsible for the conduct of those using the venue during the period that they hire the venue.
- The hirer must report to the IBA counter before using the venue.
- The hirer uses the facility at their own risk. IBA will not be held liable for any injury, illness, or loss.
- The hirer can only access the venue for the times stated and agreed upon.
- The hirer is to ensure that children and spectators adhere to the IBA terms and conditions.
- Strictly no outside catering to be brought onto the premises.
- Theming and decorations to be approved by IBA. No naked flames, including candles, are permitted.
- Any event audiovisual or technical requirements to be approved by IBA e.g. smoke machines.
- White sole or non-black marking shoes to be worn inside the arena. No high heels.
- Only water and sports drinks are permitted on the playing surface. NO food or hot drinks are permitted.
- Bouncing of balls in the arena ONLY and not in the Centre foyer/hallways.
- Walking at all times whilst in the hallways or stairwells.
- All children are to be supervised by an adult at all times.
AGREEMENT

I accept that I am fully responsible for the booking of, and payment for, the IBA stadium (or part thereof). I understand and agree to the terms and conditions for booking and hiring this facility as set out above.
