COMPREHENSIVE WAIVER AND RELEASE OF LIABILITY
FOR ALL PROGRAMS: After-School, Development Classes, Camps, Club Soccer, Indoor Futsal
READ CAREFULLY BEFORE SIGNING
IN CONSIDERATION of Nordic American Athletics (doing business as "Nordic Soccer," hereinafter referred to as "Nordic Soccer") allowing me (or my minor child/ward) to participate in any way in Nordic Soccer programs, activities, events, training sessions, competitions, camps, clinics, after-school programs, development classes, indoor futsal, club soccer, and related activities (collectively referred to as "Activities"), I acknowledge, appreciate, and agree to the following terms:
ACKNOWLEDGMENT OF SOCCER-SPECIFIC RISKS
I understand and acknowledge that soccer activities involve inherent and significant risks of injury, illness, and even death. These risks include, but are not limited to:
Physical Injury Risks:
Soccer ball contact injuries including head impacts, facial injuries, and concussions; Player-to-player contact including collisions, kicks, and falls; Goal post and equipment collision risks; Field surface injuries from falls, slides, and uneven surfaces; Cleat-related injuries from player contact; Overexertion injuries including heat exhaustion and dehydration; Weather-related risks including lightning, extreme heat, cold, and wet conditions; Equipment failure or malfunction.
Program-Specific Risks:
For After-School Programs: Risks associated with school campus facilities and property; Supervision transitions between school and program staff; Mixed-age group interactions.
For Development Classes (Ages 2-12): Age-appropriate developmental risks for toddlers through pre-teens; Equipment sizing and safety considerations; Varying skill and attention levels among participants.
For Camps: Extended supervision periods and activities; Outdoor exposure for multiple hours; Group activity management risks.
For Club Soccer & Competitive Programs: Higher intensity training and competition risks; Tournament and travel-related risks; Referee and official interaction.
For Indoor Futsal: Hard surface playing conditions; Wall and boundary collision risks; Faster-paced game dynamics.
Additional Risks:
Communicable diseases including COVID-19, influenza, and other infectious diseases; Transportation risks to and from activities; Facility-related risks including parking areas, restrooms, and spectator areas; Inadequate medical treatment or emergency response; Severe injuries including paralysis and death.
While Nordic Soccer implements safety rules, provides age-appropriate equipment, and promotes proper technique, the risk of serious injury, permanent disability, and death cannot be eliminated.
ASSUMPTION OF RISK
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE of Nordic American Athletics (doing business as "Nordic Soccer") or any of the Released Parties (as defined below), and I assume full responsibility for participation in the Activities. I understand that participation is entirely voluntary, and I (or my child) am free to discontinue participation at any time.
RELEASE AND WAIVER OF LIABILITY
I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, HEREBY RELEASE AND HOLD HARMLESS Nordic American Athletics (doing business as "Nordic Soccer"), its directors, officers, employees, agents, volunteers, coaches, officials, supervisors, managers, other participants, school districts, schools, facility owners, sponsoring agencies, sponsors, advertisers, and owners and lessors of premises used to conduct Activities (collectively, the "Released Parties"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property incident to involvement or participation in the Activities, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASED PARTIES OR OTHERWISE, to the fullest extent permitted by law.
INDEMNIFICATION
I agree to indemnify, defend, and hold harmless all Released Parties from any and all claims, demands, causes of action, damages, judgments, costs, and expenses (including attorney's fees) arising out of or related to participation in the Activities, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASED PARTIES, to the fullest extent permitted by law.
MEDICAL FITNESS AND TREATMENT AUTHORIZATION
I certify that the participant is physically fit and capable of participating in soccer activities; has not been advised otherwise by a qualified medical professional; has disclosed all medical conditions, injuries, or limitations; understands that Nordic Soccer recommends a physical examination before participation.
I acknowledge that soccer involves risk of head impacts and concussion; I will immediately report any suspected head injury to staff; participant will be removed from activity if concussion is suspected; medical clearance will be required before return to play.
I authorize Nordic Soccer, its agents, employees, and volunteers to obtain or provide emergency medical treatment; transport to a medical facility if necessary; share relevant medical information with medical personnel; make emergency medical decisions if I cannot be reached.
PROGRAM-SPECIFIC ACKNOWLEDGMENTS
For School Campus Activities (After-School Programs): I understand that activities may take place on school district property and agree to comply with all school policies and procedures; follow school district safety protocols; respect school property and facilities; understand that Nordic Soccer and school district share certain responsibilities.
For Age-Appropriate Activities (Development Programs): I confirm that participant meets the age requirements for selected program; I understand activities are designed for specific developmental stages; I will communicate any developmental concerns to staff.
For Extended Supervision (Camps): I understand that camp activities involve extended periods of supervision; activities may include various recreational activities beyond soccer; weather contingency plans may alter scheduled activities.
COMPLIANCE AND SAFETY
I agree to comply with all rules and regulations for participation; follow safety instructions provided by Nordic Soccer staff; use appropriate equipment as required; report unsafe conditions immediately; conduct in sportsmanlike manner at all times; refrain from participation if under influence of substances; arrive on time and follow pickup/drop-off procedures.
If I observe any significant hazard, I will remove myself/my child from participation and notify Nordic Soccer staff immediately.
PHOTO AND VIDEO RELEASE
I grant Nordic Soccer permission to use photographs, videos, or other recordings taken during Activities for promotional, educational, or marketing purposes, including websites, social media, brochures, and publications. No compensation will be provided.
TRANSPORTATION AND PERSONAL PROPERTY
I acknowledge that Nordic Soccer is not responsible for transportation to/from Activities unless specifically provided. If driving, I do so at my own risk. I understand that Nordic Soccer is not responsible for lost, stolen, or damaged personal property. I agree to label all personal items; use only approved equipment; inspect any Nordic Soccer equipment before use.
CONDUCT AND BEHAVIOR
I agree to conduct myself/ensure my child conducts themselves in accordance with Nordic Soccer's code of conduct. I understand that inappropriate behavior may result in immediate removal from Activities without refund. Inappropriate behavior includes unsportsmanlike conduct; harassment or bullying; violation of safety rules; disrespect toward staff, participants, or facilities.
COMMUNICATION AND TECHNOLOGY
I acknowledge that Nordic Soccer may use communication apps or platforms; I am responsible for monitoring communications; social media policies apply to all participants and families; I will not share inappropriate content involving other participants.
LEGAL PROVISIONS
This agreement shall be governed by California state law. If any provision is found unenforceable, remaining provisions shall continue to be valid. This waiver represents the complete agreement between parties.
ACKNOWLEDGMENT
I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
This agreement shall be binding upon my heirs, assigns, personal representatives, and next of kin.
FOR PARTICIPANTS UNDER 18 YEARS OF AGE
I certify that I, as parent/guardian with legal responsibility for this participant, have read and explained the provisions in this waiver to my child/ward, including the risks of soccer activities and responsibilities for following rules and regulations. My child/ward understands and accepts these risks and responsibilities.
I, for myself, my spouse, and child/ward, consent and agree to the release provided above for all Released Parties, and we release and agree to indemnify and hold harmless the Released Parties from any and all liabilities incident to my minor child's/ward's participation in these Activities, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.
