- I consent to be contacted directly by Jonathan’s Path staff.
- I will provide proof of employment and income, including a job offer letter and/or my last three (3) consecutive pay stubs.
- I agree to maintain steady employment throughout the vehicle program’s waiting and approval process.
- I have a minimum of $500 available to contribute toward the purchase of my vehicle.
- I am able to obtain and afford vehicle insurance that meets the requirements set by the state of Tennessee.
- I do not have a history of DUI (Driving Under the Influence) or DWI (Driving While Intoxicated).
- I acknowledge that submission of this application does not guarantee approval and is subject to eligibility review.
