This August Freddi is bringing a FREE football training to Te Kori Point for kids aged 8-12 years old! A fun opportunity for all skill levels to get active, develop their football skills and enjoy the game with a licensed football coach.

Time: 4.15-5.15pm
Bring: Football Shoes, Shin Pads and Water Bottle.

Informed Consent and Acknowledgement
I confirm that I am the parent or legal guardian of the child registered for the free football coaching session and give permission for them to participate in the session organised by Freddi’s Football Coaching (FFC). I understand that football and other physical activities involve some level of risk, including the possibility of accidental injury. I understand that FFC coaches and staff will take reasonable care to provide a safe and positive environment, while recognising that some injuries can occur as part of normal sporting activity. I agree that my child will follow the instructions and guidance provided by the coaching team. I will also inform FFC of any relevant medical conditions, allergies, injuries, medications, or other circumstances that may affect my child's ability to safely participate.

Medical Consent and Emergency Treatment
I authorise FFC coaches and staff to provide reasonable first aid and seek medical assistance if my child is injured or becomes unwell during the session. In an emergency, I authorise qualified medical professionals to provide necessary treatment where required for my child's health or safety. Every reasonable effort will be made to contact me or the nominated emergency contact as soon as practicable. If necessary, FFC may contact emergency services and arrange transport to a medical facility. This consent applies for the duration of the free coaching session. By registering my child, I confirm that I have read and understood this information and consent to their participation.
