Incorrect entries will delay the rescheduling process.
By checking this box you are confirming that you have contacted the opposing team. If the opposing team has not been contacted prior to the submitted request then the game will be recorded as a forfeit.*

Reschedule requests must be submitted a minimum of 7 days prior to the game scheduled date. Requests submitted within 5 days of the game date could result in game forfeiture or a rescheduling charge consisting of referee fees and assignor fee ($10). Reschedule requests shall be limited to 2 per team per season.*

Payment arrangements for late reschedule fees must be made prior to reschedule approval. Fees can be paid at the home club concession stand with prior notice.
