Mosaic Tray and Taco Night Terms and Conditions
Event Description: I understand that I am registering to participate in the “Mosaic Tray and Taco Night” event at Kalisi Cafe, located in Crofton, BC. This adults-only event, held in the main café, includes a taco buffet and an activity involving creating a mosaic on a wooden tray. Drinks are available for purchase at Kalisi Cafe. If I choose to bring my children, Kalisi Kids will provide supervision at the kids’ centre for three hours, including a taco buffet for an additional $10 per child. By signing or agreeing to this waiver, I confirm that I am 18 years of age or older and voluntarily choose to participate.
Assumption of Risk: I acknowledge that participation in this event involves certain inherent risks, including but not limited to potential exposure to allergens such as nuts, dairy, gluten, eggs, or other ingredients commonly found in tacos or other food products. The cafe and kids’ centre environment may contain traces of these allergens due to cross-contamination. I am solely responsible for reviewing ingredient lists (available upon request), disclosing any known allergies or dietary restrictions to event staff prior to participation for myself or my children, and deciding whether to consume or handle any provided items. I assume all risks associated with allergic reactions, food-related illnesses, or other health issues that may arise from participation for myself or my children.
Release of Liability: In consideration of being permitted to participate in this event, I hereby release, waive, discharge, and covenant not to sue Kalisi Cafe, Kalisi Kids, their owners, employees, agents, volunteers, or affiliates (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, injury, or illness, including death, that may be sustained by me or my children during or as a result of participation in the event, whether caused by the negligence of the Released Parties or otherwise.
Indemnification: I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to my or my children’s participation in the event, including but not limited to claims related to allergies, consumption of food or beverages, or any other activity involved.
Medical Authorization: In the event of an emergency, I authorize the Released Parties to secure medical treatment for me or my children at my expense if I am unable to do so myself.
Acknowledgment: I have read this Waiver and Release Agreement in its entirety, understand its terms, and agree to be bound by them. I sign this agreement freely and voluntarily, without any inducement.
