Pet Information, Veterinary Release and Agreement Form

TERMS
1.      I hereby authorise the attending veterinarian to provide medical treatment to my pet(s) named above and as listed on the pet information sheet.

2.      I, as the pet(s) owner, accept full responsibility for all the expenses incurred in the treatment of any pet(s) named above.

3.      I authorise Paws and Claws Neston to transport my pet(s) to the veterinary practice or to request “on-site” treatment if deemed necessary.

4.      In the case of an emergency, I understand that Paws and Claws Neston will make every attempt to contact me, and in the event I am unavailable, then my emergency contacts.

5.      In the case of an emergency and Paws and Claws Neston being unable to contact me, or the emergency contact, I authorise Paws and Claws Neston to act on my behalf to authorise any treatment, excluding euthanasia.

 

his Agreement confirms the arrangements between Paws and Claws Neston and you for the care of your pet(s), including dog walking, pet sitting, cat pop-ins and small-animal visits.

By signing, you confirm that:

🐾 Your Pet(s)

• You have provided accurate information about your pet(s), including their health, behaviour and care requirements.
• Your pet(s) are appropriately vaccinated and up to date with flea and worm treatments where applicable.
• Dogs must be registered.
• You will provide everything needed for their care, including food, medication, bedding, litter, leads, harnesses and any other necessary equipment.
• You will tell us about any medical, behavioural or safety concerns.
• You will tell us if you do not want your dog to be let off lead in a safe and appropriate environment.

🏡 Visits & Access

1. We will always aim to arrive at the agreed time, but please allow a one-hour window for visits, pick-ups and drop-offs.
2. Any keys, access codes and personal information provided to us will be kept secure and confidential.
3. Please let us know if anyone else will have access to your home while we are caring for your pet(s).

💳 Payment & Cancellations

• Services are paid for in advance.
• Pet sitting requires a 50% deposit to secure your dates.
• Late payments may incur a £20 charge.
• Dog walks cancelled with less than 24 hours' notice may be charged in full.
• Pet sitting cancelled with less than 7 days' notice will incur a charge of 50% of the scheduled visits.
• If you return home early during a pet sitting booking, the full agreed fee remains payable.

🚨 Emergencies
If you cannot be contacted, you authorise Paws and Claws Neston to make reasonable decisions regarding your pet's care. You remain responsible for any veterinary or emergency costs and for additional emergency care time, charged at £20 per hour.

 

🛡️ Safety & Insurance
Paws and Claws Neston is insured for pets in our care. However, we cannot accept responsibility for incidents that could not reasonably have been prevented. You remain responsible for any damage, injury or costs caused by your pet(s).

 

📷 Photos & Social Media
We may take photographs or videos of your pet(s) and use them on Paws and Claws Neston social media or for promotional purposes.

Please let us know in writing before services begin if you do not give permission.
