Custom Tattoo Form
After submitting this form i will get back to you within 1-3 days with a price estimate and we can discuss when you want to schedule your appointment! Please submit the form at least 1 week before you plan to get the tattoo so I have enough time to draw up the design. Once I get the deposit I will send you the design the day before your appointment and I am willing to make any changes you would like to make. I require a $40 deposit for the appointment that DOES go towards the final price, as well as an additional $20 custom fee that does NOT go towards the price as this is to compensate for my time to draw your design!
