Custom Tattoo Form
After submitting this form I will get back to you with a price estimate and we can discuss when you want to schedule your appointment! Please submit the form at least 1 week before you plan to get the tattoo so I have enough time to draw up the design. I require a $50 deposit for the appointment that DOES go towards the final price, as well as an additional $30 custom fee that does NOT go towards the price, as this is to compensate for my time to draw your design! Once I finish your design I will send it to you and am happy to make as many changes you'd like!
