  • By signing at the end of this Agreement, I acknowledge that I have read all six pages of this Registration Agreement as set forth below and agree to abide by all terms, conditions and waivers.

    I hereby release and discharge The Edge School of the Arts Dance Legacy, the Jamaica Center for Arts & Learning, and any Artists in Residence (including but not limited to KECDE!, Reform LIVE, Milan Jolie Enterprises, Life Camp), and their respective owners, directors, management and staff (for the purposes of this agreement hereafter referred to collectively as ESOTA) from and against any and all liability or causes of actions arising out of, or in connection with, my enrollment in ESOTA or my participation in any of its functions. Throughout this agreement, JCAL refers to the Jamaica Center for Arts & Learning and the Jamaica Performing Arts Center.

  • IN CLASS ATTIRE

    • ESOTA requires all students to dress professionally in class at all times.
    • All students should carry hand sanitizer and wipes in their dance bags.
    • Hair should be neatly pulled back off of the face. Beads, ribbons, dangling barrettes, and loose hair are distracting, dangerous, and not allowed. Buns or hair puffs are required at all times.
    • All students must wear the appropriate Uniform (see the Class Uniform List) for all regularly scheduled classes.
    • No extra clothing is to be worn in class and will be removed from students. If temperatures are cool, students are allowed to wear a plain black T-shirt, sweat-shirt, dance pants or sweatpants, or approved ESOTA dancewear. All dancewear must be labeled with the student's name INSIDE THE GARMENT in case it is misplaced.
    • All students, except Babies, must have a dance bag that is large enough to hold all personal belongings, dance shoes, and street clothes and must carry it with them at all times. NO SHOES, BAGS OR CLOTHES MAY BE LEFT IN DRESSING AREA OR HALLWAYS. All bags must accompany students in their classroom. All items are expected to be in the student's dance bag, except for their coat.
    • Lockers are available for daily use. Locks must be removed daily. Any locks not removed will be cut and items emptied. Lockers are not to be shared by students unless they are immediate family members who live in the same household.
    • NO JEWELRY it is dangerous and can be lost. Any jewelry worn in class will be removed from the student and ESOTA cannot and will not be held responsible for its loss or damage.
    • Students MUST arrive wearing their dancewear under their regular clothing.
  • LOST AND FOUND

    • Lost and Found items will be held in the Business Office for a limited time. On the last Saturday of each month, the Lost and Found will be cleaned out and all items (except for shoes and jewelry) that remain will be discarded or donated.
    • It is imperative that all Waiting Areas, Hallways, Classrooms and Dressing Areas are kept clear of clutter. In an attempt to keep them clear and teach children the responsibility of ownership, any items left lying around unattended will be confiscated and placed in lost and found.
  • SECURITY CAMERAS

    • For purposes of safety and security, JCAL has installed video surveillance cameras throughout the building in classrooms, hallways, and common areas, except restrooms.
  • ATTENDANCE/LATENESS/ABSENCE

    • Students are expected to arrive 10 minutes before their first class. Any student who is more than 10 minutes late will have to sit and observe class and the lateness will be considered an absence.
    • Excessive absence may result in demotion or dismissal. Any student who is deemed to have missed too many classes to keep up with their class will not be permitted to participate in the June concert.
    • In case of injury, students are expected to observe class.
    • All students are expected to attend extra scheduled rehearsals (without additional cost) as concert time nears. Babies Class Members must wear their ID cards at all times. There is a $5 fee for the replacement of lost name tags.

     

  • ARRIVAL

    • Upon entering JCAL, all students/parents/visitors should stop at the ESOTA Desk. Babies will be escorted upstairs by an ESOTA Representative. All other students are to proceed upstairs to the dance classrooms.
    • Students must arrive fully dressed for class.
    • Babies' parents should take all clothing items with them when leaving JCAL.
    • All students must arrive wearing their dancewear under their regular clothing. Clothing should be conducive to quick and easy changing. i.e. sundress, sweatsuit, slides, etc.

     

  • WAITING & PICK UP

    • There is a small waiting area available in the Gallery on the first floor of JCAL. Please adhere to our "drop and go' policy. No parents (except for approved Class Assistants) are permitted upstairs, except when authorized by ESOTA Management or in cases of an emergency.
    • Parents are to drop off their children and pick them up ON TIME when their classes are over. For NEW Babies only - until children have adjusted (approximately 1 month), parents should stay in the neighborhood so we can reach you by cell phone in case they need your support.
    • It is imperative that parents and guests understand that under no circumstances should you interrupt any class or rehearsal in progress - no matter the reason. Parents and Guests are not allowed in dance rooms or in areas designated for "Students Only unless invited by the teacher, with the approval of management. Teachers and students are not to be distracted. Any issues should be addressed to ESOTA's Desk.
  • PICK UP/DISMISSAL

    • All Students will be brought down to the lobby area by an ESOTA Representative. ESOTA reserves the right to charge a late pick up fee for students that are repeatedly picked up late.

     

  • PAYMENTS

    • ESOTA will be open every day of the season, EXCEPT for the following dates (this schedule subject to change):
      • Thanksgiving Break: Thursday, November 27 through Saturday, November 29. 
      • Winter Recess Break: Friday, December 26 through Saturday, January 3

      • Mid-winter Recess: Monday, February 16 through Saturday, February 20

      • Spring Recess: Monday, April 6 through Saturday, April 10

      • Memorial Day – Friday, May 22 – Saturday, May 23

    • Updates to any holiday closures will be provided through email.
    • When in doubt about closures, call ESOTA and check the voicemail OR visit our Facebook page. PEP Students will have rehearsals or performances during the Winter Recess Break and possibly during the Midwinter Recess break.
    • Space is reserved for each child on a seasonal basis. Students register for and are responsible for paying for the entire season. Your season begins with your first day of classes, and ends either on the last day of classes at ESOTA, or on the day you notify ESOTA's Front Desk in writing that you are dropping. You are responsible for payments for the entire season, even if you are late, observing or absent from class. This includes absences due to illness, travel, birthday parties and extra-curricular activities for self, child or family member.
    • ALL students must pay a non-refundable and non-transferable registration fee for the season as follows:
      • Early bird registration is $50 and takes place through September 7.
        Pre-registration is $60 and takes place September 8 – September 30.
      • General Registration is $65 and takes place from October 1 - October 31.
      • Late registration is $75 and takes place from November 1 - January 31.
      • Last chance Registration is $100 and goes into effect on February 1.
    • ALL students must pay a non-refundable and non-transferable costume deposit of $50. This fee is due upon registration.
    • We offer three payment options for your convenience.
      • ANNUAL - Payment in full is due upon Registration. A 10% discount off the Class Fees will be provided for those choosing this option if you register by October 12.
      • QUARTERLY - Installment payments are due in October, December, February, and April
        • STANDARD ENROLLMENT STUDENTS will pay on the 1st Saturday of those months.
        • PEP/PRO CLASS STUDENTS will pay on the 2nd Saturday of those months.
        •  A 5% discount off Class Fees will be provided for those choosing this option if you register by October 12. All late payments accrue a $10/week Late Fee. Additionally, you will forfeit your discount and will be transferred to the Monthly Payment Plan.
      • MONTHLY
        • STANDARD ENROLLMENT STUDENTS will pay on the 1st Saturday of each month. ALL ACCOUNTS MUST BE PAID IN FULL BY THE FIRST SATURDAY OF EACH MONTH. Any accounts not PAID IN FULL by the first Saturday of the month will be assessed a Late Fee of $10 per week until the account is brought up to date. These Late Fees will continue to accrue until either the day the account is brought up to date, or the day you notify the Business Office in writing that you are dropping.
        • PEP/PRO CLASS STUDENTS will pay on the 2nd Saturday of each month. ALL ACCOUNTS MUST BE PAID IN FULL BY THE SECOND SATURDAY OF EACH MONTH. Any accounts not PAID IN FULL by the designated Saturday of the month will be assessed a Late Fee of $10 per week until the account is brought up to date. These Late Fees will continue to accrue until either the day the account is brought up to date, or the day you notify the Business Office in writing that you are dropping.
    • Payment for lessons should be made electronically via Zelle. Payments can be made in person via cash, money order, or check. There is a $25 fee for any check returned for non-payment. ESOTA reserves the right to revoke check writing privileges for any student account due to a returned check. Checks must be dated and funds available on the same date the desk transaction is being made. Post-dated checks are not accepted. Checks will not be accepted for payment of tickets, late fees, journal, costumes, pictures, etc. Check payments will not be accepted after May 9.
      • Zelle payments are to be made to esotacustservice@cs.com.
    • ESOTA's Front Desk must be notified immediately, in writing, if a student will no longer be attending class, or is changing their schedule by dropping or adding a class. You are responsible for payment on all lessons up until the day that you notify ESOTA's Front Desk in writing or send an email of any changes to esotacustservice@cs.com.
    • If you choose to withdraw your child from ESOTA, you'll be financially responsible for the amount due through the end of the month that you withdraw. All payments made prior to the date of withdrawal, including pre-payments for costumes, lessons, pictures, etc., are non-refundable and non-transferable. They can only be applied to future credits for the current student.
    • Students whose accounts are more than 2 months behind will be removed from our roster and not permitted to take class. Accounts must be brought up to date in order for a student to be reinstated. The Reinstatement Fee is $50.
    • Students are not allowed to sign up for extra activities, performances and/or Field Trips with ESOTA unless accounts are up-to-date.
    • Students who leave ESOTA with an account in bad standing will not be permitted to register again until the past account is paid.
    • Payments & questions regarding your account must be settled in person on Saturdays or electronically. We cannot handle phone or email inquiries about your account. ESOTA's Front Desk Hours are Saturdays from 10 a.m. to 5 p.m. ESOTA's Front Desk is open at 9:45 a.m. to allow students to dress, please understand that we will not be ready to supervise students until 10 a.m.
    • In the event that an in-person scheduled class/rehearsal has to be changed to a virtual class/rehearsal, the payment schedule and fees will remain the same.
  • JUNE CONCERT

    • All students are expected to participate in the end-of-year concert, which will be held one weekend in June. If any reason you cannot participate, you must notify ESOTA's Front Desk ASAP. Parents should prepare to purchase and sell the minimum amount of tickets required per family, pay for costumes, purchase accessories and may want to participate in the commemorative journal. There will be a meeting and/or detailed meeting notes to discuss specifics as concert time draws near.
    • A non-refundable non-transferable $50 costume deposit is due upon registration.
    • Children under 3 are not permitted to attend the June Concert.
    • Vaccination status of guests will be determined by the Executive Orders in place at the time of the Concert. Performance attendees must adhere to the health guidelines that are in effect during that time.
    • Each family, as defined by legal residence, is required to purchase a designated number of tickets to the June Concert. The final number will be determined by the time of the annual parent meeting and will consider any governmental mandates at the time of the meeting.
    • Full and non-refundable costume payment will be required as scheduled during the months of February/March. You may, and we strongly recommend that you begin making small down-payments on costumes as early as you wish.
    • No costumes will be handed out without full payment on your account, your costumes and your ticket balance.
  • RESPIRATORY ILLNESS RELATED CLOSURES & PROTOCOLS

    • All students, parents, staff and visitors agree to abide by ESOTA’s HEALTH SAFETY POLICY. By registering, you are acknowledging that you have read the plan and agree to its terms.
    • Should an extended health-related emergency require programming to shift from in-person to remote, you will be notified immediately.
    • If needed, classes will be conducted virtually.
    • Virtual classroom etiquette rules will be provided should the need arise to shift to remote learning due to any state of emergency issued that prevents in-person classes from occurring.
    • A unique login and password will be provided to registered students.
  • RULES & REGULATIONS

    • There is no talking during dance class.
    • No student is allowed to leave class without the teacher's permission.
    • Parents/family members are only permitted to use the bathrooms on the first floor.
    • Parents/family members are not permitted in any classroom.
    • Elevators are reserved for the use of ADULTS. Children must use the stairs, unless a medical condition requires such.
    • Only dance shoes will be permitted on dance room floors. Dance shoes should NOT be worn in the street!
    • There is NO GUM chewing allowed in the building.
    • There is NO EATING OR DRINKING allowed in the building. Children, Teens, PEP students and Adults may take bottled water into class. Food and beverages discovered will be disposed of, and ESOTA will not be responsible for replacing.
    • JCAL is a cultural habitat for artists. Visual art is displayed throughout the facility. Please respect the artists and their work. Anything broken or damaged by a student, parent or guest must be paid for by the parent or guardian of record.
    • Students (with the exception of Teens, Junior Senior Pro and Aspring Dancers) are not permitted to leave the building without adult supervision. In the rare instance where an exception must be made, parents must provide written consent to ESOTA's Front Desk before permission can be granted.
    • Students are not permitted to use the conference room on the 2nd floor, unless directed to do so by ESOTA Management.
    • Students, parents and guests are not permitted to videotape classes, rehearsals or performances without prior expressed consent of management.
    • The recording and/or posting of video footage of classes, rehearsals or performances on the internet or social networking sites by students, parents or guests without expressed consent of management is strictly prohibited.
    • Cell phone use during class is forbidden. In the case of an emergency, a student should notify their teacher of any concerns and ESOTA's Front Desk will contact a parent/guardian if needed to resolve the issue. All cell phones should be placed on vibrate during class. Phones are an unnecessary distraction to the classroom.
    • There is no soliciting (to include but not limited to distributing flyers, fundraising, candy, cookie, or other like item sales), of any kind without prior approval of management.
    • Students are expected to demonstrate model behavior at all times inside and outside the classroom. If your child has been removed from a class for disciplinary reasons, you will be asked to come pick them up immediately.
    • Babies and Children must enroll in consecutively scheduled classes. If there is a break in instructional time,a parent/guardian must pick up their child during such break.
  • COMMUNICATION

    • ESOTA always welcomes customer comments and suggestions. Please email us at esotacustservice@cs.com.
    • ESOTA's Front Desk is located in the lobby of JCAL. Please make sure to advise ESOTA's Front Desk of any changes of address, email address, phone numbers or authorized pick up list. It is important that we be able to reach you in case of emergency It is imperative that you attend our Virtual Parents' Meeting held as scheduled in January or February.
  • CONDITIONS FOR DISMISSAL

    • Any student (or parent, guardian or guest of any student) discovered rifling through someone else's property or destroying property belonging to ESOTA or ESOTA's students, staff and guests, will result in the immediate dismissal of the registered student without refunds of any monies paid.
    • ESOTA reserves the right to dismiss or deny registration to any student whose self, parent, guardian, relative, or guest behaves in a manner not consistent with the standards of ESOTA and/or will not or cannot follow the rules and regulations set forth by ESOTA (whether set forth herein or otherwise widely acknowledged), without refunds of any monies paid.
  • VACCINATION

    • At the present time, there is no vaccination requirement in effect.
    • Should a NYC Executive Orders mandate any such change to that status based on a health emergency, we will provide you with updated information immediately.

     

  • ADDITIONAL WAIVERS

    • I hereby authorize ESOTA to act for me according to its best judgment in any emergency requiring medical attention and acknowledge that any special medical conditions that I am aware of have been noted on my Registration Form and that I am responsible for the payment of such costs.
    • I hereby authorize ESOTA to photograph and videotape and to use my likeness (or that of my child or charge), image and performance from any ESOTA owned (or taken) photographs or videotapes for any display, publication, journal, broadcast, distribution, advertising or promotions relating to the school and its programs without remuneration.
    • I hereby acknowledge the rule that no valuables or jewelry should be worn at ESOTA, the requirement that students carry a bag large enough to carry their personal belongings, including footwear, and that said bag is kept in their classroom and/or possession, and in so doing acknowledge that ESOTA cannot be held responsible for any lost or stolen articles or monies.
    • I hereby acknowledge that participating in physical activities at ESOTA, including classes, rehearsals, and performances, carries some risk of injury. I accept this risk for myself, my child, and any family members or guests while on the premises or attending ESOTA events. I understand and agree that ESOTA cannot be held responsible for any injuries that may occur.
    • I hereby acknowledge that by participating in classes, rehearsals, or activities at ESOTA, there is a risk of being exposed to illnesses such as colds, flu, COVID-19, or other communicable diseases. While ESOTA takes steps to reduce these risks, no measures can completely prevent exposure. I accept this risk for myself, my child, and any family.
    • I hereby agree to notify ESOTA should my child's school be closed due to a COVID-related outbreak.
    • I hereby acknowledge that I have received and signed ESOTA's COVID-19 waiver.
    • I hereby acknowledge that ESOTA must adhere to any Executive Orders or mandates issued by the City of New York and that I will be apprised of any adjustments to scheduling required as they are issued.
