The following are the names of individuals that have your permission to pick up your dog(s) from Playful Paws Day Camp. Anyone picking up a dog that is not an owner will be required to show ID. You understand that once dogs are released into the care of the authorized person(s) or individuals, Playful Paws Day Camp is no longer responsible for the well-being of your pet.

It is recommended that your dog has his/her breakfast at home prior to coming to play at Playful Paws Day Camp. Treats and water will be provided throughout the day while playing at Camp. Please check if you are allowing our staff to give treats to your dog(s) while their stay with us.

Playful Paws Day Camp will offer Nail Trims while your dog stays with us. There
will be a fee of $12-$15 for a Nail trim. Please ask for the consent form at our front desk if you would like this service.
This Waiver shall be in effect unless otherwise revoked in writing by the
Owner. A current waiver must be on file for the dog(s) to attend day camp or
any other programs offered by Playful Paws Day Camp.

I represent that I am the legal owner or authorized by the owner of the dog(s) in the Day Camp Program.

I confirm that if my dog(s) is 8 months or older, they have been either Spayed (female) or Neutered (Male).

I understand that when participating in Day Camp, my dog(s) may be required to wear a short lead leash hooked to a collar to monitor behaviors.

I represent that my dog(s) is in good health.

My Dog(s) is free of fleas, ticks, and lice and on a monthly preventative.

My Dog(s) has not been ill in the last 30 days.

My dog(s) is current on all listed vaccinations and will present the proper paperwork from a veterinarian to have on file for the following vaccinations: *Canine Distemper *Rabies *Bordetella (Kennel Cough)

*For everyone's safety, rabies vaccines must be given by a licensed vet. No backyard boosters, please only paw-fessional care will do!

I understand that while my dog(s) is/are fully vaccinated, that vaccines are not guaranteed and there is a small risk that dog(s) may contact a contagious disease or illness. I agree that should this occur, I am responsible for my pet care, medical attention and costs.

I release Playful Paws Day Camp, owners, its staff, agents, and representatives, as well as any other dog owners, from all liability related to injury, sickness, damage, or death resulting in participation in day camp or other day camp programs.

I understand that although all dogs are supervised, incidents of injuries may occur from playing with other dogs, which may include but not limited to bites, scrapes, scratches, and sprains. I waive any claims against Playful Paws Day Camp, any of its staff, owners, agents, or representatives, or owners of other day camp participants for any claims relating to any injuries incurred by my dog(s) while participating in the day camp program.

I represent that my dog(s) is/are appropriately socialized with both people and other dogs and have not harmed or shown threatening behaviors towards any person or other dog. I understand that Playful Paws Day Camp reserves the right to remove my dog (s) from the play area and place my dog(s) in a separate holding area should my dogs display any unwanted behaviors. I further understand that I may be asked to pick my dog(s) up early from day camp in the event that my dog(s) exhibits any of these behaviors and that I will not receive a refund for the day.

I hereby authorize Playful Paws staff to contact my veterinarian should any injuries or illness require medical attention. I agree I am solely responsible for any medical expenses incurred for my dogs.

In the event that I fail to arrive by the time Day Camp closes to pick up my dog(s), Playful Paws is authorized to contact my Emergency Contact to pick up my dog(s). I understand that my emergency contact must show identification to pick up my dog(s) and that I may be charged additional fees for failing to arrive prior to closing time.

I agree to pick up my dog by 5:30pm from Day Camp. The owner will understand that he/she may be charged a late fee of $1.00 for every minute late and not picked up by the designated check out time.

Social Media Release Form

This form is giving consent for the caretakers at Playful Paws Day Camp to take pictures of your pet while in their care and can upload them to all social media platforms. The photos taken will belong to Playful Paws Day Camp and will be used for marketing purposes.
Playful Paws Day Camp platforms are:

• Instagram
• Webpage

Although the images will be Playful Paws Day Camp, in the event you want an image removed it will be removed, and you do not have to provide a reason.

I hereby give permission for Playful Paws to take images of my pet whether they are on the bus or at the place of business and use them for social media.

Our live cams are a fun way to check in on your furry friend during the day! To make sure this feature stays safe, respectful, and enjoyable for all pet parents, please follow these simple rules:
🐾For Enrolled Clients Only: Access is granted only to families with dogs currently enrolled in daycare.
🐾One Account Per Family: Please do not share login info with others.
🐾Viewing Hours: Cams are available during active play hours (typically 8:00 AM – 4:00 PM, Mon–Sat).

Usage Guidelines
🐾No Recording or Screenshots: For privacy, please do not record or post footage online.
🐾Be Kind & Respectful: Our team works hard; please refrain from making assumptions or criticisms based on short clips. If you have concerns, contact us directly.
🐾No Remote-Control Features: Cameras are view-only for safety and security.

Security & Tech
🐾All footage is streamed securely and not stored.
🐾Camera access may be paused during maintenance or staff-led training.
🐾Misuse of camera access may result in suspension of live cam privileges. We’re here to help!

Every Dog Deserves a First Class Ride!

Playful Paws Day Camp cares deeply about the wellbeing of its canine guests and looks after them to the best of its ability while seeking to provide the best possible care. However, I understand that dogs can sometimes be injured or become ill during transportation and understand that every dog reacts differently and that animals, by nature, are unpredictable. Therefore, for myself, my heirs and assigns, I have read, understand, and agree to the following:

🐾 I understand that my dog will be an established pet at Playful Paws Day Camp by having at least one play day at Camp prior to being able to ride the bus.

🐾 I hereby waive, release, and discharge Playful Paws Day Camp and its owners, agents, officers, employees, or volunteers from any and all liabilities that result in injury to, or the loss of my dog(s), or any other property of mine which arises in any way out of the transportation provided by Playful Paws Day Camp.

🐾 I hereby agree to defend, indemnify, and hold harmless Playful Paws Day Camp, its owners, agents, officers, employees, or volunteers from and against any and all claims of any nature including all costs, expenses and attorneys’ fees, which arise in any way out of the transportation provided by Playful Paws Day Camp.

🐾 I understand that Playful Paws Day Camp will be providing transportation for my dog(s). Playful Paws Day Camp agrees to exercise reasonable care of my dog(s) during transportation. I acknowledge that allowing Playful Paws Day Camp, its owners, agents, officers, employees, or volunteers to transport my dog(s) involves risks of injury to my dog(s) during transportation. I knowingly accept and assume all known and unknown risks in allowing my dog(s) to be transported.

🐾 I recognize the risk of injury that accompanies transportation of my dog(s) and acknowledge that this release is being relied upon by Playful Paws Day Camp to permit transportation of my dog(s) to and from Playful Paws Day Camp located at 150 Auburn Road Branson, Missouri 65616, or any other necessary location such as a home or a designated pick up and drop off location.

🐾 I accept any and all conditions, rules, and regulations proclaimed by Playful Paws Day Camp, associated with the activities, use of the Playful Paws Day Camp facilities and transportation, and hereby agree to comply with them.

🐾 Playful Paws Day Camp will take precautions to prevent dogs from running away from the bus or property but dogs’ behavior cannot be perfectly controlled at all times. I will not hold Playful Paws Day Camp responsible if my dog finds a way to flee their premises during care or the bus during transportation.

🐾 Playful Paws Day Camp may, at any time, remove a dog from transport, or refuse a dog transportation, if the dog exhibits behavior that could be harmful to the dog, other dogs, or staff, such as:

🐾 Aggression or uncooperative behavior;

🐾 High anxiety or fear;

🐾 Refusal of dog to load into vehicle;

🐾 A condition or behavior rendering dog generally not fit for transportation;

🐾 If, in the opinion of Playful Paws Day Camp employee or agent, transportation is not in the best interest of dog.

🐾 I understand that dogs can sometimes be injured or become ill during transportation. I accept that Playful Paws Day Camp is not in any way responsible for any injury, stress or illness my dog(s) may suffer while under Playful Paws Day Camp are or while being transported by Playful Paws Day Camp. I will not hold Playful Paws Day Camp liable, financially or legally, for any physical, mental, or health issues that arise during or as a result of my dog’s transportation by Playful Paws Day Camp. This includes, but is not limited to: dog bites, minor cuts and scratches, bone breakage, tissue damage, joint problems, stress, or diarrhea.

I, the undersigned, affirm that I am freely signing this Waiver and Liability Release. I have read this Waiver and Liability Release and fully understand that by signing this Waiver and Liability Release I am giving up legal rights and/or remedies which may otherwise be available to me regarding any losses I may sustain. I agree that if any portion is held invalid, the remainder will continue in full legal force and effect.

Home Entry Agreement

As part of our convenient pickup and drop-off bus service, we may need to enter your home to retrieve or return your dog. The following terms are to ensure a safe, respectful, and smooth experience for both parties.

By signing below, you authorize Playful Paws Day Camp staff to enter your home solely for the purpose of:

• Picking up or dropping off your dog for daycare services.
• Placing your dog in a designated area (e.g., crate, gate, or safe room) after services are complete.

Safety & Responsibility

• Your dog must be in a secure, agreed-upon area and ready for pickup at the scheduled time.
• Playful Paws Day Camp is not responsible for property damage or pet injury caused by unsecured pets, unlocked doors, or unsafe conditions in or around your home.
• Please notify us of any alarms, cameras, or special instructions we should be aware of.

Liability & Confidentiality

We respect your privacy and your property. Our team will:
• Never enter any area not necessary for pet pickup/drop-off.
• Lock up as instructed and leave your home exactly as we found it.
• Maintain full confidentiality of your access codes and personal information.
