• Annual Letter of Informed Consent

  • Dear Parent:

    As part of our GNCV Youth Ministry, we plan and schedule annual events and activities for a time of fellowship and community building. As such, we request your permission for you child to participate in these supervised activities listed below.  Should you wish for your child to not participate in the activity, please formally let the leaders know in writing via email (info@goodnewschapelvan.org) prior to the scheduled event. The safety of your child/children is our primary concern. Precaustions will be taken for their wellbeing and protection.

     

    GNCV Youth 2026 Activities

    1. Hay rides or animal-driven rides of any kind;
    2. Boating, fishing, surfing, water tubing, ice skating, canoeing, kayaking, snow skiing,
    snowboarding, snow tubing and other activities or events on water or snow;
    3. Zip lining or trampoline parks;
    4. Bonfires, camp fires or open fires;
    5. Outdoor camping with or without the use of tents;
    6. Swimming and other activities in bodies of water other than guarded public pools;
    7. Carnivals (Such as PlayLand, Fair at the PNE);
    8. Corn mazes, pumpkin patches and similar activities;
    9. Mechanical rides/devices or water parks;
    10. Go Karting or any other motor vehicle races, competitions, demonstrations or shows
    11. Rodeos;

  • Communication

    A policy is in effect that communication is to be used solely for the dissemination of information. Please sign below to grant permission for Youth Ministry Personnel (staff and volunteers) to communicate with your youth via telephone, email, social media and text:

  • I hereby consent to the participation of my/our child/children in any of the supervised
    activities listed above for the year 2026. I understand that should I wish for my child to not participate in the activity I will formally let the leaders know in writing via email prior to the scheduled event.


    I acknowledge that photographs and or video recordings can be taken at these events for the purpose of social media and the church website and I grant permission for my child to be in photographs and or video recordings.

    While every precaution is taken for the safety and good health, some sports and activities carry with them the inherent risk of personal injury beyond the risks associated with many of the recreational activities at Good News Chapel. I/we understand and accept these risks and agree that by allowing my Child to participate in those activities, he/she may be taking part in a recreational activity that presents the potential for personal injury.


    I/we, the Parents or guardians named below, authorize the Pastor or one of Good News Chapel personnel to sign consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above.

    I/we, named below, undertake and agree to indemnify and hold blameless Good News Chapel, its personnel, its leaders and Board against any loss, damage or injury that is suffered by the participant as a result of being part of the activities of Good News Chapel, as well as of any medical treatment authorized by the supervising individuals representing Good News Chapel. This consent and authorization is effective only when participating in or traveling to events of Good News Chapel.

    I have read, understood and agree with the above and hereby give permission for my son/daughter to travel and attend GNCV Youth activities with the leadership of Good News Chapel.

    I take full responsibilities for my actions and will pay for anything I damage. I will abide by the guidelines and code of conduct. I will do my best to display a godly character and attitude.

  • Parental Consent for Participants Under the Age of 13

  • Our youth and young adults program is designed for individuals ages 13 to 30. We recognize that, on occasion, children under the age of 13 may wish to participate. By signing below, I acknowledge that my child is under the age of 13 and does not fall within the recommended age range for this program.


    I confirm that I have spoken with the youth leader and that permission has been granted for my child to join though they are under the minimum age. I acknowledge that participation in the Program involves certain inherent risks. I give permission for my child to attend and participate in the group’s activities. I understand that the program content, discussions, and environment are primarily geared toward participants 13 and older. I accept responsibility for allowing my child to join and release Good News Chapel BC, its leaders, volunteers, and representatives from any and all liability, claims or demands that may arising from my child’s participation in the program.
