VEHICLE REPAIR AUTHORIZATION

1. Authorization for Appraisal, Dismantling, and Repair: I, the undersigned, hereby authorize Porcelli's Auto Center to appraise, dismantle, and repair the vehicle described above. I understand that dismantling my vehicle may be necessary to fully assess the extent of the damage and to prepare an accurate estimate for repairs. I also authorize Porcelli's Auto Center to operate my vehicle for testing, inspection, and/or delivery purposes as needed during the repair process.

2. Acknowledgment of Insurance Appraisal Limitations: I acknowledge that the appraisal provided by my insurance company may not fully reflect the labor rates, products, and services offered by Porcelli's Auto Center. I understand that the insurance company's appraisal is an estimate and may differ from the actual costs incurred during the repair process.

3. Authorization to Adjust Repairs: I authorize Porcelli's Auto Center to proceed with the necessary repairs to my vehicle, using their professional judgment to determine the most appropriate materials and methods. This includes the use of OEM parts where required and the use of higher-quality paint and materials to ensure the best repair outcome. I also authorize Porcelli's Auto Center to adjust the repair plan as necessary to account for differences between the shop's posted labor rates and those prescribed by the insurance company. I understand that these adjustments aim to ensure my vehicle is repaired to the highest industry safety standards.

4. Standard of Repair: I understand that Porcelli's Auto Center is committed to repairing my vehicle to the highest industry safety standards, ensuring that all work performed meets or exceeds safety and reliability requirements.

5. Authorization to Begin Repairs: I authorize Porcelli's Auto Center to begin repairs on my vehicle as soon as my insurance company inspects the vehicle and provides an estimate, without any further delay.

6. Acceptance and Signature: By signing below, I acknowledge that I have read, understood, and agree to the terms of this Vehicle Repair Authorization Agreement. I accept the terms outlined above and authorize Porcelli's Auto Center to proceed with the repair of my vehicle as described.

PENDING LIABILITY 

DISCLAIMER Please Read Carefully By signing below, I acknowledge and agree to the following:

1. Pending Liability on Insurance Claim: If the liability determination on my claim by the other party's insurance company remains unresolved for more than three (3) days, I hereby give Porcelli's Auto Center permission to proceed with processing the claim through my own insurance company for the necessary repairs and rental vehicle coverage.

2. Responsibility for Rental Costs: If I choose not to use my own insurance coverage and the other insurance company has not yet accepted liability, I agree to return the rental vehicle and pick up my car. I also understand that I will be responsible for covering the cost of the rental vehicle for the duration of time it was in my possession.

AUTHORIZATION AND PAYMENT AGREEMENT

1. Authorization for Repairs: I, the undersigned, hereby authorize Porcelli's Auto Center to perform all necessary repairs on the vehicle described above. I acknowledge that the standard repair rate is $80.00 per hour and the frame repair rate is $80.00 per hour.

2. Review of Contract: I understand that I have the right to review this contract before signing. I have been informed that all parts used for the repair of my vehicle will be factory-recommended replacement parts and procedures unless otherwise noted in this agreement.

3. Guarantee of Repairs: I acknowledge that all authorized repairs will be fully guaranteed to meet or exceed industry standards for safety and performance. Porcelli's Auto Center is committed to ensuring that the repairs on my vehicle are performed to the highest standards.

4. Payment Terms: I agree to make payment in full for all repairs directly to Porcelli's Auto Center at the address provided above. If payment is not made within 30 days of the billing date, I agree to pay an interest rate of 1.5% per month on any unpaid balance. Furthermore, I agree to cover all attorney's fees and other costs incurred by Porcelli's Auto Center in the collection of any unpaid amounts.

5. Legal Compliance: I acknowledge that all repairs and services provided by Porcelli's Auto Center will be carried out in accordance with Rhode Island General Laws, as they may be outlined. 

6. Acceptance and Authorization: By signing below, I authorize the repairs to my vehicle as outlined in this agreement and agree to the terms and conditions set forth. I confirm that I have read and understood this agreement in its entirety and accept all terms.

DIRECTION TO PAY AND OEM PARTS

The undersigned hereby acknowledges that the repairs or replacement for the loss and damage to the insured/claimant's vehicle have been completed to their full satisfaction. By signing below, the undersigned agrees that the payment of the sum determined in the estimate provided by the insurer directly to Porcelli's Auto Center. This constitutes full performance and discharge of the insurer's obligations

Request for OEM Parts Only: I hereby request that only Original Equipment Manufacturer (OEM) parts be used in the repair of my vehicle under Rhode Island law, R.I. Gen. Laws § 27- 10.2-2(c), which states:

(c) No insurance company may refuse the use of OEM parts when negotiating repairs with any repairer for automobiles that are greater than forty-eight (48) months and less than seventy-two (72) months from the date of manufacture, provided the repairer has written consent from the vehicle owner to install OEM parts.

By signing , I acknowledge that I have read and fully understand all of the terms, conditions, disclaimers, and authorizations outlined above. I agree to comply with these terms and authorize Porcelli's Auto Center to proceed accordingly.
