    MINOR CONSENT AGREEMENT

    I have read the script and have no problem with my child playing the content as written or with the changes indicated in the script after the read through.  I am willing to allow my student to participate in this process.  If I have any concerns I will contact the director, but otherwise will make sure that my student is present at all required rehearsals and work days and participates in set break-down and clean-up after the production. 

    I understand that for the safety of my child, I will enter the building to pick up my child, making sure that the director or producer acknowledges that I am picking up my child.

    RULES AND REGULATIONS

    I agree to follow the rules and regulations outlined in this document.

    I understand that I will be replaced if I have 3 unexcused absences from the first meeting to the last performance. Absences must be cleared by the director prior to absence. It is my responsibility to notify the director at least 3 days prior to being absent from rehearsal. In case of an emergency, an exception may be granted.

    I will attend all rehearsals and arrive at least 15 minutes prior to call time unless a conflict has been pre-approved by the director. If you are more than 10 minutes late to rehearsal, without prior approval, this will be marked as an unexcused absense.

    I will bring to every rehearsal my script, a pencil, a pad/paper for notes, any pertinent production materials and be in proper dress.

    I will not bring any food or drink into the theater or backstage areas. I will make sure that anyone attending a rehearsal with me (i.e., parents, family members) is aware of this rule as well.

    I agree not to use my cell phone or other electronic devices during rehearsals or performances, unless I have received director approval. Furthermore, I aggree to leave my electronic device put up and put on silent durning all rehearsals and performances. The director, and other authorized crew members, reserve the right to take my electronic device until the end of rehearsal if I have it out during a rehearsal or performance. My parent/guardian will contact the director prior to rehearsal if a circumstance comes up where I will need to have access to my phone.

    I agree to replace, rebuild, or pay for any costume, prop, script, set piece damaged, lost, or destroyed by my misuse or mishandling.

    I agree to respect all the performers, backstage workers, and volunteers who are working on the production. This theatre is a NO DIVA ZONE. We are a family who will work together to create a fantastic show.  I understand that as a cast member, I am a representative of Good Times Community Theatre League, and will represent it well.

    I understand that the use of tobacco products (including any type of vapor product), alcohol, or any other illicit drugs will not be tolerated. The use of these may be terms for immediate removal of the production.

    I agree to assume other duties that may rise and are assigned to me. I will follow all directives and wishes of the director.

    I have read, understand, and freely sign this contract. I further understand that should my behavior jeopardize the good of the production (i.e. tardiness, unexcused absence, illegal acts, or disrespect). I will receive a verbal warning, followed by a written warning at the second incident. A third infraction will lead to me being asked to leave the production and possibly my removal from the next production I audition for, or the rest of the season.

    I agree to release Good Times Community Theatre from all liability relating to injuries that may occur during rehearsal, work days, and/or productions. By signing this agreement, I agree to hold Good Times Community Theatre entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

     

    *Parent/Guadian* As the parent/guardian of a minor in this production, I aggree to ensure my child will uphold the rules as laid out in the contract above. Should I, or my child, break these rules; I understand that the cast member will be held responsible. I will ensure my child will be on time and ready to work.

    MEDIA RELEASE

    The Good Times Community Theatre League periodically receives attention from various media asking to take photographs and/or video of events, rehearsals and programs.  This exposure is a great benefit to Good Times as a whole, and brings understanding of the critical need for the arts in our area. We ourselves take pictures and/or video to use on our website, Facebook page, and other social media; and in our camp videos, press releases, flyers, and various other forms of advertising.  

    We need your permission, or in the case of a minor, a legal guardian’s permission, to do these things. We ask you that read and respond to the information below.  If you choose not to grant permission, this will in no way affect your opportunity or any child’s opportunity to participate in Good Times Community Theatre productions or events.

    SAFE THEATRE POLICY (ANTI-HARASSMENT)

    Good Times Community Theatre League is committed to providing a safe and harassment-free environment for its members, volunteers, community partners, and board of directors. Our organization refers to “harassment” as unwelcome conduct that is based on race, color, religion, sex (including sexual orientation, gender identity, or pregnancy), national origin, age. While GTCTheatre is committed to handling all forms of harassment as quickly as possible, we do know that it is impossible to provide a document that outlines repercussions for every form of harassment. Due to the complex nature and ever-fluid definition of the term “harassment,” the following sub-policis have been established in an attempt to mitigate any form of harassment that may happen throughout our organization. While these sub-policies exist, the currently sitting board of directors reserves the right to handle all harassment cases brought to them on a case by case basis. This policy can, and will, be updated as deemed neccasary according to the GTCTheatre Board of Directors.

    (1) Complaint Policy

    All forms of harassment must be reported in order to ensure that sufficient attempts of resolution are made on behalf of GTCTheatre. We have established a 3 step escalation hierarchy to handle all harassment cases.
    1) If a member of our organization feels they are being harassed, a verbal complaint may be brought to the Director(s) of the current production. The Director(s) reserve the right to resolve the issue as they see fit. 2) If a member of our organization feels they are still being harassed after a verbal complaint has been made to the Director(s) of the current production, a formal complaint may be brought to the Director(s) of the current production. A formal complaint form can be retrieved from our website or by request. This form must be completed and turned in to the Director(s) of the current production. The Director(s) reserve the right to resolve the issue as they see fit. A summary of the resolution must be added to the formal complaint and submitted to the sitting board of directors for retention purposes. 3) If a member of our organization feels they are still being harassed after a formal complaint has been filed, a formal complaint may be submitted to the sitting board of directors. Depending on the severity of the complaint, resolution will be determined through special meeting, or at a regularly scheduled board meeting.

     
    (2) Infraction Retention Policy

    Repeated harassment will not be tolerated by the Good Times Community Theatre League organization. The sitting board of directors reserves the right to apply any stipulations to a member who is known to be a repeat offender. However, due to our current non-existent nature of harassment, a retention policy will not be put in place until the time such a system is required. The sitting board of directors reserves the right to update this policy as time progresses.


    (3) Base Layer Costume Policy

    Good Times Community Theatre is open to every member of our community regardless of age, race, religion, gender, sexual orientation, etc. To ensure the comfort and protection of our members, a policy will be established to ensure cast members should be covered at all times during performances. A BASE LAYER of clothing to be worn under the participants costume so that during costume changes backstage they are never naked. We do not have access to private dressing rooms, so this provides a way for everyone to change costumes backstage. 

    (4) Dressing Room Policy

    Good Times Community Theatre is open to every member of our community regardless of age, race, religion, gender, sexual orientation, etc. To ensure the comfort and protection of our members, Good Times is committed to providing up to 3 dressing rooms during productions requiring more than one costume change. Dressing room designations will include: (1) Male, (1) Female, and (1) Unisex affixed with private changing stalls. In addition to backstage dressing rooms, quick change stalls may be requested by any actor on a production-by-production basis. Actors requesting an off stage quick change stall must give the director at least 48 hour notice to comply.


    (5) No Phone/Camera Policy

    Good Times Community Theatre will maintain a strict no photos in dressing rooms policy. This includes (but is not limited to) times of rehearsal, pre-performance, performance, and post-performance. Directors reserve the right to allow phone usage during productions, however directors do not reserve the right to allow usage inside dressing rooms. If phone usage is allowed during a performance, designated “safe zones” should be established prior to production.
