Acknowledgements
Purpose of Massage: I understand that massage therapy is intended to promote relaxation, relieve muscle tension, and improve circulation. It is not a substitute for medical examination, diagnosis, or treatment.
Licensed Therapists: Services will be provided by a qualified massage therapist employed or contracted by the Palo Alto Massage Therapy Center (MTC).
Parental Presence: I acknowledge that I may remain present in the treatment room if I wish, and that MTC encourages open communication with me and my child about comfort levels during the session.
Boundaries & Comfort: My child will be appropriately draped at all times. Massage will only be provided to areas agreed upon beforehand. My child may ask the therapist to stop the session at any time.
Medical Conditions: I have disclosed any relevant health information about my child, including injuries, allergies, or medical conditions that may affect their ability to receive massage.
Liability Waiver: I release MTC, its therapists, and staff from any liability for injury or adverse reaction that may occur as a result of massage therapy, except in cases of gross negligence or misconduct.
Consent
I hereby consent to my child receiving massage therapy at MTC. I understand the nature of the services and give permission for massage to be performed on my minor child.
