By allowing us or your school/team to use one of your selected favorite photo(s) from your gallery, you’re helping us showcase the best moments from each event. These highlights may be featured in our Community Group or as a Photo of the Week to inspire athletes and celebrate their hard work. Your support also gives us the opportunity to share your/your athlete’s success story while strengthening the spirit of the school and team.

NOTE: checking one or more of these boxes does not guarantee that your favorite photo will be selected for promotional use. Final choices are at the discretion of AJ’S SPORTS GALLERY LLC and/or your school/team.

By signing & selecting "Agree" on this form, I confirm that:

• The information I have provided is true, accurate, and complete to the best of my knowledge.
• I am the athlete, or the parent/guardian of the athlete if under 18 years of age, and I have the right to book this session.
• I understand that AJ’S SPORTS GALLERY LLC photographs athletes only at events, practices, or sessions where permission has been granted by the school, team, or event organizer.
• I acknowledge that all galleries are private and passcode-protected, and that any reference photo I upload will be used only for identification and deleted after verification.
• I understand that promotional use of photos will only occur with express permission provided by the athlete (if over 18) or their parent/guardian (if under 18).
• Group Photo Release: I understand that if the athlete is part of a group photo, you consent to the photo being viewable and purchasable by the other athletes pictured and their parents/guardians.
• I understand that these images are for personal use only and agree not to share my gallery passcode publicly.
• I understand that AJ’S SPORTS GALLERY LLC is not responsible for inaccurate information provided in this booking form and reserves the right to decline or cancel the booking if details cannot be verified.

1. Select your package (starting at just $20).
2. Confirm your event date & time.
3. Re-enter your contact info for booking confirmation (Parent's contact info this time, if client is under 18).
4. Submit your booking request.

Once reviewed, you’ll receive an email letting you know if your booking has been accepted or declined.

NOTE: All galleries are private and passcode-protected, and the extra photos can always be purchased within your gallery.
