Westside Community Market Participation
Acknowledgment of Risks:

I, the undersigned, acknowledge that my participation in the Westside Community Market involves certain risks, including but not limited to personal injury, property damage, and other unforeseen risks. I understand that these risks may arise from my own actions or inactions, or the actions or inactions of others, and that I am voluntarily assuming these risks.

Release of Liability:

In consideration of being allowed to participate in the Westside Community Market, I hereby release, waive, discharge, and covenant not to sue Ivory Bay Community Development Corporation, its officers, employees, agents, and volunteers from any and all claims, liabilities, losses, or damages that may arise in connection with my participation in the event, including but not limited to any injury or damage to my person or property, whether caused by the negligence of the Releasees or otherwise.

Indemnification:

I agree to indemnify and hold harmless the Releasees from any and all claims, damages, or losses, including legal fees and expenses, arising out of my participation in the Westside Community Market.

Medical Treatment:

In the event of an emergency, I hereby consent to receive medical treatment deemed necessary by the event organizers or emergency personnel. I understand that I am responsible for any medical expenses incurred as a result of such treatment.
Understanding and Agreement:
I have read this Liability Waiver and Release Form and fully understand its contents. I am aware that this is a release of liability and a contract between myself and the Releasees. I sign it of my own free will.
