FAMILY LIFE ASSEMBLY OF GOD/ BOLTZ VOLLEYBALL SPONSOR RECIPIENT The Boltz Volleyball Club will be Sponsored for the 2026-2027 club season. These sponsorships will cover the cost of tournament fees, facilities fees, coaching fees, practice fees, and coaching traveling fees with Boltz Volleyball Club. This does not include commitment fee, travel to and from and lodging for all tournaments. Players must try out and make a Boltz volleyball team for the 2026-2027 season, to be considered for the sponsorship, and will not be released early from the team. Completing this application does not mean you will be awarded a sponsorship. There may be partial sponsorship/ Financial assistance available throughout the season as well.This sponsorship application form must be submitted to the director of the Boltz Organization by August 17th for Boys and November 29th for girls. The recipient(s) will be notified, By September 1st (Boys), the first week of December (Girls). Your participation and submission of this application is strictly confidential and will not be made public. These applications will be reviewed only by the sponsorship committee made up of Family Life Assembly of God church members . Players who have completed the application process will be responsible for the cost of the commitment fee dues at the time of commitment.If you do not get accepted for a sponsorship and have signed a commitment fee you are still required to complete the payment plan as outlined. If you cannot commit to the payment plan do not accept a spot on the team by signing the financial commitment form.Recipient Requirements: All recipients selected for this sponsorship are subject to the following requirements.-Must participate in all Boltz Fundraising Events-Must stay in good standing with the club organization and commit to the team for the entire season/post season-Must follow all rules outlined for players in handbook-Must pay the due at tryouts, non refundable commitment fee. Players who do not meet the above outlined requirements may have their sponsorship revoked and all fees associated with their team will be do at that time. This sponsorship application form must be submitted to the director of the Boltz Organization by August 17th for Boys and November 29nd for girls.
