  • LICENSE & VEHICLE INFORMATION

    As a smaller school, Heritage does not have regular access to a school bus.  Thus, when transporting students for field trips and sporting events, it is necessary for us to utilize parent volunteers.  This actually provides a great opportunity to build community among Heritage families.  If you are planning on being a driver for any school activity, field trip or sporting event this year, for the protection and safety of our students, we need you to complete this form.  We cannot allow drivers without proof of license and minimum insurance requirements.  Thank you for volunteering to assist Heritage!

  • INSURANCE

    Liability limits of the policy must meet or exceed:  $100,000 each person/$300,000 each occurrence and property damage of $50,000.  (HCS’s insurance company requires that drivers provide a copy of the “Declaration” sheet of your auto insurance to HCS in order to confirm the above information and confirm coverage dates.)

  • CONDITIONS/RESTRICTIONS:

    By volunteering to drive for HCS events, you certify that:

    • The vehicle capacity is one passenger per seat belt.  All passengers in your vehicle will use their seat belts.
    • Children under 4’9” must use a booster seat and children under 13 years of shall ride in the back seat in the vehicle where it is practical to do so (see RCW 46.61.687).
    • The vehicle is in safe operating condition.
    • You have no physical limitations that would adversely affect your ability to drive safely.
    • Your cell phone will be used only in the case of an emergency and not while operating a moving vehicle.
    • You are not taking any medications that would adversely affect your ability to drive safely.
    • You have no prior convictions for driving under the influence of drugs or alcohol.

  • INFORMATION CERTIFICATION:

    I certify that the information given on this form is true and correct to the best of my knowledge. I understand that as a volunteer driver, I must be 21 years of age or older, possess a valid driver’s license, have the proper and current license and vehicle registration, and have the required insurance coverage in effect on any vehicle used to transport students. I also agree to abide by all traffic regulations, as well as any school regulations governing appropriate driver safety including, but not limited to the ones listed above. Heritage Christian School leadership reserves the right to withhold event driver privileges at its discretion, as deemed necessary for the safety, order, and best interests of the school community.

     

    Please be aware that as a volunteer driver, your insurance would be primary should any incidents arise.
