  • 1.) CANCELLATIONS & MAKE-UPS
    Cancellations should be made at least 24 hours in advance of a scheduled session. Sessions cancelled less than 24 hours in advance will be charged in full. However, clients are allowed up to 2 make-up sessions for missed appointments. If the client has already paid in full (weekly, monthly, or package), no additional charge will be applied for those make-up sessions.

    2.) LATE ARRIVALS
    Each session is scheduled for 1 hour to 1 hour and 20 mins max. If a client is running late, a 10-minute grace period is allowed. Sessions will not be extended beyond this window unless additional time is available.

    3.) CLIENT INFORMATION & RESPONSIBILITY
    All information provided on this form is true and accurate to the best of my knowledge. I have sought and followed any necessary medical advice. I understand that all information given will remain confidential.
