All Trainees in the University Hospital Sharjah hold a position of trust relative to the employees and the Hospital as a whole and must recognize the responsibilities entrusted to them in preserving the security and confidentiality of information.
Therefore, in this regard:
I, the undersigned, acknowledge that I understand and agree to adhere to the following statements:
• I am familiar with the University Hospital Sharjah HR/ Education Dep policies and procedures related to patient information, Disease, treatment terms and conditions, and Patients' history. This also allies to the release of information concerning employees, administration, and maintenance of employee records- both physical and electronic.
• I will use patient-related information for official business only in UHS and Training.
• I will not divulge any information to other parties except in the conduct of their work assigned by the University Hospital Sharjah policies. I will also not knowingly be involved in making false, inaccurate, or misleading entries.
• I will report security and privacy violations.
• I understand that all information contained in the Education Department and Training in UHS is considered confidential and is not to be shared, communicated, or made accessible to anyone unless authorized by the Education Department Manager.
• I understand that violation of this agreement may lead to disciplinary action consistent with the Hospital’s policy on discipline management.
