    I, {name}, hereby confirm that the information provided in this application is accurate. I agree to abide by the rules and regulations set forth by the Midsummer Fair organisers and undertake to:

    1. Only offer for sale crafts or produce which have been made in Cannock Wood and Gentleshaw or which I, as a resident of Cannock Wood and Gentleshaw, have made myself.

    2. Arrive on site through the Village Hall car park on Buds Road between 9am and 10am. 

    3. Bring everything needed for displaying my crafts or produce on the table safely.

    4. Park any vehicle in the parking area at the lower end of the field and not bring a vehicle onto the show ground. If needed, bring a trolley.

    5.  Be available on the 'Totally Locally: made in Cannock Wood and Gentleshaw' stall for selling my craft or produce throughout the period from 11am to 4pm or arrange cover with other tableholders, accepting that any losses are at my own risk.

    6. Not pack up the table before 4pm but then pack up promptly and leave the show ground between 4.30pm and 5pm.

    7. Take away all everything brought and waste (including empty boxes and litter) and leave the table as found.

    8. If allocated a table or half table make payment in full for this within 14 days of being sent confirmation and payment details quoting the confirmation reference. If the organisers cancel the fair or any outdoor parts of the fair, for example due to extreme weather conditions, I accept that the table booking fee may be repaid and that in no circumstances will the organisers accept responsibility for any consequential losses. 

    I understand that the submission of this application does not guarantee a place on the 'Totally Locally: made in Cannock Wood and Gentleshaw' stall, and table allocation will be at the discretion of the organisers.

    I accept full responsibility for preparation of the risk assessment for my table or half table and for operating with complete regard for health and safety of the public, other tableholders and organisers. (An example of a risk assessment is on the stallholders page of the website).

    I have provided information including personal data for the purposes of this application and agree that the Midsummer Fair organisers may retain this data in administration of the fair and will not share this information with any third party without my consent, unless required to do so by law.
