This is a Tattoo Request Form to book with Cassie Noble at Triple J’s Collective in Whitehorse.

Please fill out everything to the best of your abilities. You will hear from Cassie VIA email for any follow-up questions, unless you state another preference. Please CHECK YOUR JUNK MAIL! You will then be contacted by Triple J’s staff by phone to confirm an appointment and discuss your deposit.

Cost is $200/hr, cash only. Deposit required.

A cash deposit (minimum $100, more for larger pieces) is required to book your appointment. You can come into the shop to make your deposit, or for clients far away, you can use a credit card. The deposit is non-refundable and will be applied to the total cost of your tattoo.
