Other Service Requested

1.Winter Storage - September 1 to May 31.

2.I hereby authorize the work described on page 1 to be done along with the necessary materials, and hereby grant my permission to operate the boat, motor(s) and trailer (on water and/or over the road) as necessary for the purpose of said work (including testing and/or inspection).

3.I agree that Cape Fuel Injection, LLC dba Blue Water Boats shall have a lien on my boat, motor, and trailer to secure the amount of repairs and/or storage charges and fees which shall remain in force and effect until payment in full of all charges. This lien gives Cape Fuel Injection, LLC dba Blue Water Boats the right to possession of my property until payment in full.

4.I agree that Cape Fuel Injection, LLC dba Blue Water Boats is not responsible for any loss or damage to said boat, motor(s), trailer and contents resulting from vandalism, theft, fire, wind, rain, ice, part failure, collision, act of God or any cause beyond their control. I understand it is my responsibility to maintain a valid marine insurance policy including liability coverage on all unit(s) stored with Cape Fuel Injection, LLC dba Blue Water Boats.

5.A service charge of 1.5% per month will be added to all balances not paid within 30 days of invoice date..

6.I agree to be personally responsible for all attorney fees incurred by Cape Fuel Injection, LLC dba Blue Water Boats to collect any outstanding balances due.

7.Cape Fuel Injection, LLC dba Blue Water Boats cannot be held accountable for freezing damage caused to boat, motor(s) and/or trailer made available for pick-up or dropped off after November 15.

8.Winterization of fresh water accessories is mandatory for all boats stored by Cape Fuel Injection, LLC dba Blue Water Boats.

9.Due to insurance regulations no independent contractors, service organizations or outside individuals will be allowed to perform work or enter the facility without first obtaining permission. Additionally, all contractors, organizations or individuals will be required to provide Cape Fuel Injection, LLC dba Blue Water Boats with a certificate of insurance coverage. Said certificate shall include coverage for liability, worker's comp and name Cape Fuel Injection, LLC dba Blue Water Boats as additional insured.

10.No customer supplied parts. Also unless requested in advance, all parts replaced on boat, motor(s) and trailer will be disposed of upon completion of work.

11.Customers not performing engine tune ups will incur a de-winterization fee of $100 + Parts per engine.

12.All boats not scheduled for launch by May 31 will be charged for summer storage.

13.Summer Storage - May 1 to September 1. - $35 per ft.

14.Boat pick up - boats here after 10 days from notice of competition will be charged a storage fee of $10 a day

15.Storage customer acknowledges that vessel is fully covered by insurance and the insurance company know the boat is here

16.I authorize Cape Fuel Injection, LLC dba Blue Water Boats to charge my credit card number listed below for any invoice not paid upon completion of the work performed or within 10 days of receipt of invoice, included charged incurred during the season.
