General Rules of Behavior Expected of Each Student
1. Students must participate with the group when attending any youth event (we do not permit wandering off).
2. Our youth group activities are phone-free (unless a student needs to make contact with a parent for any reason). During Sunday night youth group and other youth events, we reserve the right to collect student phones and other devices so they are able to focus and fully participate in the group. Leaders will be accessible by phone should parents need to reach us or their child.
3. Students must respect all CBC staff, volunteer leaders, and church/venue property.
4. Any other unforeseen activity CBC staff deems inappropriate may be grounds for dismissal from that youth event and/or future youth events.
Activity Release Statement
Please acknowledge consent with the following statements:*

As parent/legal guardian of the aforementioned student(s), I have reviewed the information about the youth ministry activities/events and give my permission for the subject of this release to be involved in all activities for CBC Youth during the year 2025 as indicated above.I/We have reviewed the rules of behavior expected of each student and agree that the subject of this release will abide by them. I/We also acknolwedge that if the subject of the release is required to return home early due to inappropriate behavior, it will be at my/our expense.I/We understand all reasonable safety precautions will be taken at all times by Community Bible Church and its agents during the events and activities. I/We understand the possibility of unforeseen hazards and know the inherent possibility of risk. I/We agree not to hold Community Bible Church, its leaders, employees, and volunteer staff liable for damages, losses, diseases, or injuries incurred by the subject of this form.
Photo/Video Release Statement
Community Bible Church requests permission to use, distribute, and/or display photographs or videos taken of youth at CBC Youth events for promotion purposes during the year 2025, including but not limited to the church website.*

I/We consent to Community Bible Church using, distributing, and/or displaying photographs or videos of the subject(s) of this release in church or youth group promotions, including but not limited to the church website.I/We do not consent to Community Bible Church using, distributing, and/or displaying photographs or videos of the subject(s) of this release in church or youth group promotions, including but not limited to the church website.
CBC Youth Transportation Policy
Community Bible Church does not allow any adult youth leader to transport any student in CBC Youth in a car without either one or more additional approved adults in the car or one or more additional students in the car.
Transportation Release Statement
Community Bible Church reserves the right to designate approved adult youth leaders and parents to transport groups of minor students in a car to and from CBC Youth events.*

I/We consent to Community Bible Church designating approved adult youth leaders and parents to transport the subject(s) of this release with other minor students in a car to and from CBC Youth events.I/We do not consent to Community Bible Church designating approved adult youth leaders and parents to transport the subject(s) of this release with other minor students in a car to and from CBC Youth events.
