      Acknowledgment of Terms and Conditions: I understand that the Team is required to follow all USA Hockey and CAHA policies during travel. If applicable, I agree to follow CAHA policy 10.3.2.10, stating that only 10U AA teams can participate in 10U AAA or 10U AA tournament divisions. 10U A teams can participate in 10U AA or 10U A divisions. 10B may participate in 10U A or B. I have also read policy 10.3.5.2 and agree to abide by this rule. If applicable, I agree to follow CAHA policy 10.4.2.9, stating that only 12U AA teams can participate in 12U AAA or 12U AA tournament divisions. 12U A teams can participate in 12U AA or 12U A divisions. 12B may participate in 12U A or B. I understand that any Game Misconducts and/or Match Penalties must be reported to the CAHA Disciplinary Chair within 24 hours of receipt of the infraction. I understand/agree that if playing a team not registered with USA hockey or Hockey Canada that all participant families will need to fill out/sign the unsanctioned play waiver, the team will need to abide by USAH and CAHA rules for unsanctioned play, and that these games will not count toward any year-end game counts. I understand that it is the responsibility of the team manager or head coach to verify if opposing teams and/or tournaments are sanctioned by USAH or Hockey Canada. If it is discovered that a tournament or opponent is not sanctioned by USAH or Hockey Canada, I understand the team must immediately inform the Affiliate Registrar and request permission to compete. I understand it is the responsibility of the team manager or head coach to monitor the number of games played and level those games are at. An approved travel permit does NOT imply CAHA approval for going over game counts, travel limits, or playing games at an unapproved level.
