Please complete this registration form to enroll in the Awana program @ First Baptist Church, 310 E I Ave, Grundy Center, IA 50638. Ensure all information is accurate and complete. Books are $15 for each student, Club dues are $15 for 1 year per student, or students may bring .50 each week instead. Payments can be made on the first night, which is Wednesday, September 10, 2025. Program runs 6:30 - 8:00 p.m. each Wednesday night. Books will be passed out on the first night, or you can stop by the office Tuesday - Friday; 9:00 a.m. - Noon & 1:00 p.m. - 4:00 p.m. to pick up in advance.
